UOPX Memo To: Care Team From: Corin Stanley cc: Attending physician and medical team Date: 05/06/2022 Re: Lewis Good day team, Today we have Lewis who was recently bought into the ED due to excessive abdominal pain. Lewis was diagnosed with Crohn's disease. I am sure you all know but for those who do not, Crohn's disease is chronic inflammatory bowel disease that affects the lining of the digestive tract. Lewis is 21 years old and is anxious after receiving his diagnosis. I want all of us to provide Lewis with the best care as well as information to help him cope with this disease. Our attending physician will begin Lewis on steroidal medication to help slow down the progression. Lewis will begin the medication twice a day while here in the hospital and then a prescription will be sent to his local pharmacy. When administering the medication, the patient must have a full glass of water and it must be taken with food. Special dietary orders have been placed. Lewis is aware that he cannot consume any alcohol, coffee, dairy products, or carbonated beverages. I have sent Lewis’s file to our dietitian on call and they will visit Lewis prior to him being discharged to discuss his new meal plans. Since the disease was discovered early, we have high hopes to help Lewis get back to his daily life as soon as possible. With that being said, we are still to provide the highest level of care. Crohn's disease has had an impact on society. Due to the conditions and symptoms of the disease, one may experience mental health issues such as depression. Loss of appetite and rapid weight loss. I have provided Lewis with contact information to support groups, individuals who have the same diagnosis and/or other life changing diseases.
