Report
9:2 Final Project Presentation of findings
Data Visualization As the Quality Manager at Happy Health Hospital, I am presenting the comprehensive findings of our root cause analysis on patient falls over the past four quarters. This investigation delved deep into trends, patterns, and contributing factors to inform and support data-driven policy recommendations that prioritize patient safety.
prevalence I used the stacked column and stacked bar to demonstrate the difference in days from admission date to the documentation date for both the fall risk and vitals signs data points. This demonstrates the common date and time structure while presenting the viewer with the difference in days for documentation to take place (Oachs, & Watters, 2020). I utilized a pie chart for the medication review to demonstrate the percent that each medical professional would review the medication so it’s easy to compare and see how often the review is not being done by authorized personnel (Oachs, & Watters, 2020). An area comparison chart was used to demonstrate the date and time from admission to when the medication was reviewed to present the difference in time from admission to review and how long for that to take place (Oachs, & Watters, 2020). It’s just a different view from the stacked column and bar charts.

Patient Demographics These graphs help HIM professional analyze which areas of charting and documentation need improvement to become compliant within the regulating bodies standards. With the information regarding the length of time from admission to the fall risk assessment the HIM professional can see that it is taking several days for this documentation to take place. The standard for the Agency for Healthcare Research and Quality (AHRQ) is upon admission within the first 24 hours and following any change in patient status or location changes (Agency for
Patient Demographics From the evaluation of the data presented in the stacked columns it shows that this is not occurring and the HIM professional will need to evaluate why and what process, policies, or both need to change to be compliant again. Having information in a visual graphed form makes it easier for the HIM professional to evaluate the areas that need improvement and address the issues in a more timely manner providing faster results.
Fall Risk Documented Date & Time Please see attached word document for graphs. My apologies but I couldn't figure out how to get them on the powerpoint slide. I had to create them via excel.

Recommendations Take the lead in highlighting how important it is to lower the number of injuries caused by falls. Create an interdisciplinary team or assess the current team's makeup to reduce falls-related injuries. Employ a tried-and-true technique to determine the variables that raise the chance of falling. create a personalized care plan based on the patient's recognized fall and injury risks, and deliver treatments in accordance with the patient's needs. Organize and put into practice tactics and therapies that have been proven effective (Joint Commission, 2015).
