Fundamentals – Lec (Week 3) Identifiers – always 2 Sentinel event – an adverse event leading to death and serious physical or psychological injury Electrical safety – patients are not supposed to bring their electronics from home (microwave, hot plate, etc) Seizure precautions – padded bed rails, suction, O2 (risk for aspiration) Fire: PASS – pull, aim, squeeze, sweep (side to side) RACE – rescue, alarm, contain, extinguish Infection control – HANDWASHING Restraints – rule of 2’s: check every 2 hours, document every 2 hrs, space around restraint should be about 2 fingers wide, padding over the bony prominences, tie around the bedframe NOT the siderails, remove immediately if there are s/s of circulation issues (gray/purple skin, etc) then inform provider; offer diversional activities to redirect patients prior to applying restraints Patient has fear of falling – call light, night light, offer assistive devices, offer physical assistance Confused patients – put them in a room near the nurse’s station for close monitoring IVs and lines – cover them up first before resorting to restraints Bathroom safety – rails, commode Dementia patient – bed alarm or chair alarm Fall risk – bathroom urgency (if patient is on diuretics), confusion, medications, weakness Infection control – types of precautions, PPE, handwashing C-diff – wash hands with soap and water, contact precautions Needlestick injury – 1) rinse/wash first , 2) report to supervisor Signs/symptoms of localized infection – redness, swelling, warm to touch Systemic infection – abnormal vital signs, lab values Pain – assess pain level and location to determine nursing intervention, document thereafter Electronics – safety issue especially around a patient who uses O2 Complete VS – location of the where the pulse rate and BP was taken, method (radial, apical, manual BP or electronic), document thereafter Subjective vs Objective data – review
