Derek
Brief summary of why the child was referred and the problems he was having in school.
Derek is a fourth grade student who repeated 3rd grade. He has difficulty in spelling, reading, and writing. He also needs to build his attention and focus skills while becoming an organized learner. Staying focused and organized are a struggle for Derek even with supports such as sitting within close proximity to the teacher and one-on-one instruction. Derek has significant scores in the areas of impulsive-hyperactive, learning problems, anxiety and hyperactivity scales. Goal # 1 Derek will be able to read a passage and answer 40 out of 45 word and passage comprehension questions correctly for 4 out of 5 trials. Strategies:
Derek will read passages of interest to him such as sports. Each passage will include a variety of text features including graphics, charts, pictures, bold fonts and color coded text. Derek will have passages on tape (audiobooks) to listen to as he reads independently. Derek will also
have the ability to request a passage be read to him along with the independent reading.
Derek will peer read passages with a friend to help collaborate on the passage read while building rapport with other students. Derek will also have the opportunity to read passages in a small group with 2-3 peers and a collaborative discussion. This goal will be completed by April 25, 2022. Assessments will be given on a weekly basis through observation, work samples, and alternative means to assess passages (i.e. drawing a picture for a summary, playing a game, giving an oral report, etc…).
Goal # 2 Derek will be able to write at least a 60 word passage, using 51 unique words and spelling 45 words correctly for 4 out 5 trials. Strategies:
Derek will work in a small group within the general education classroom to work on spelling assistance. Support will be scaffolded as improvement occurs through weekly assessments. A variety of activities such as computer games, word games, and manipulatives will be used. Derek will be able to use a variety of techniques to remember spelling such as pneumonic devices, rhymes, and songs to help with new spelling words. Prompts of correct grammar such as capitals and
punctuation will be relayed to him before a writing passage and posted on his desk. Derek will take all written spelling assessments in print handwriting with the ability to free write a sentence in cursive. Derek will also take use a variety of assessments such as fill in the blank worksheets, writing words in a preferred medium, and alternative methods such as using sensory techniques to write his words (shaving cream, kinetic sand, magic erase boards, etc…).
This goal will be completed by April 25, 2022. Assessments will be given on a weekly basis through observation, work samples, and alternative means to assess spelling (i.e. sensory bins, matching cards, computer typing, etc...).
Goal # 3 Derek will use provided strategies to become an organized and focused learner. He will use provided organizational strategies and prompts for 4 out of every 5 assignments. He will be able to focus on his work for 60 minutes with two prompts or less.
Strategies:
Derek will be given the choice between 2 organizational strategies (i.e. graphic organizers, thinking maps, concept maps, outlines, scaffolded notes, etc…) before starting a new project. He will be given short prompts to keep his attention on his organizer and offered discreet keywords (
“Ready... Set... Hike!”
) when he has gone off task. Derek will be given the opportunity to have a short break (3-5 min) after 20 minutes of focused work
time. These breaks may include a walk to the water fountain, getting to read a book of his choice in the reading nook or drawing a picture of the current classroom story. After 20 minutes of focus for 4 weeks, the focused work time will be extended 1.5 times with the final goal to be 60 mins to achieve
his break. Breaks will be adjusted to reflect Derek’s current interests with a focus on academic, movement or sensory activities. Derek will be given a dedicated spot to organize his work. There will be color-coded folders for him to put his work into. This spot will be close to his desk and he will be able to get up to get or put away necessary materials for the subjects we are working on. Each ELA, Social Studies, or Science long term project will have a rubric for Derek to check for materials before handing the assignment in. This goal will be completed by April 25, 2022. Assessments will be given on a weekly basis through observation, timed samples, and alternative means to assess focus and organization strategies (i.e. requesting a specific graphic organizer, recognizing meeting a goal, reduction in focus prompts, etc...).
