S.O.A.P. Report
Client Name: Abby Decker
Social Worker’s Name: Angel Nichols
Date: February 5, 2024
Meeting was from 2:00 pm to 3:00 pm
Subjective: Abby states that after she was put on ADHD medicine, she can concentrate more in school and thus stays out of trouble while there. She said she no longer feels her home is a safe environment for her Will and her. Abby reported that her mom’s boyfriend Diego has been abusing them and performing corporal punishment on them when he is drunk. She stated he is drunk most nights and becomes very mean. Abby said last night she came in from school and was five minutes late. Diego was drunk and as soon as he seen her, he went up to her and smacked her in the mouth and told her she was late. Objective: Abby seems more edgy and fidgety than in previous sessions. She has this overall scared and helpless demeanor about her. Assessment: While Abby is now attending all her classes since she was put on ADHD medicine, the underlying problem is Diego and the abuse. According to Abby, because he abuses her, she does not respect him nor like him. She is more worried about her brother Will being safe than she is of her own accord.
Plan: Due to Abby reporting abuse in the home, a Child Services Report will be made, and they will manage the situation from there. If possible, Abby will continue her sessions throughout the process.

I have been working with Abby for about 6 months now as her social worker. She was referred to me by her homeroom teacher. We have been working on coping skills and doing better in school. In her last session, Abby advised she feels unsafe in her home due to abuse occurring. Abby is a 15-year-old female that lives in Wichita, KS. She was diagnosed with ADHD shortly after receiving services and is being medicated with 30 mg of Adderall per day. Abby has one sibling, named Will who is 12 years old. Beth (Mom) has been dating an illegal immigrant named Diego for 3 years, who has an expired travel visa. Beth and Diego cannot afford to marry
due to all the legal paperwork that would have to be done. Abby states that Diego abuses her and Will as well as uses corporal punishment on them when he is drunk. Abby advised that on 2/4/24 she came home from school and was only about five minutes late. Diego took one look at her, walked up to her, and smacked her in the mouth as he mumbled that she was late coming home. Abby stated this is not the first time Diego has abused her or Will. Diego lives in the home and has made it to where Abby does not feel safe living there and would like to find a safe place for her and Will to go. Abby and Will are being deprived of the basic need to feel safe in their environment. If measures are not taken to find these kids safety, the fear is that they will get hurt worse the next time the abuse occurs. They are already suffering mentally and emotionally.
