Imagine you are an Epidemiologist at local department of public health. You received a call from Dr. Wilson, the Medical Director for a local nursing home facility, who reported 60 elderly residents exhibiting symptoms of vomiting and diarrhea. She was worried the residents were exposed to a stomach virus or have food poisoning. Dr. Wilson provided you with the laboratory test results and asked you to perform an epidemiology investigation to determine the source of their illness. a. What type of study would you use and why? Your hypothesis is the residents became ill from a meal served within the past 48 hours. The suspected meal included grilled chicken, potato salad, biscuits, cake and ice tea. You are not 100% sure which item caused the illness but with consideration of the laboratory result you suspect the chicken caused the illness. For this outbreak I would use the cohort study, retrospective system. I would be able to collect data that is readily available to determine a level of exposure that the residents have been exposed to. I would use the information I received from Dr. Wilson along with the information regarding the menu, lab results, and the period of exposure and symptom onset. b. What is the attack rate of those who eat chicken? Attack Rate = Ill/Ill+Well x 100 during time period AR = 35/35 + 25 x 100 = 58.3% c. ***Using the information in the data below, calculate the proper measure of association (odds ratio or relative risk) for the chicken using a Fourfold table (also called a 2 X 2 table). Ill Yes No Total EXPOSED Yes A=35 B=10 A+B=45 No C=5 D=10 C+D=15 d. Do you have enough information to claim the chicken is the cause of the residents’ illness? Why or why not? No, the chicken is not the cause of the illness, otherwise people that did not eat the chicken would be fine, however there are some sick with the illness that did not eat the chicken.
***Ill/Ill+Well. There are 60 residents. If 35 are ill then 25 are not. 35+25 = 60. 10 points is a bit extreme for every wrong thing, should I quit this course because it is impossible? I will talk to my advisor about dropping it.
