The reasons for the problem faced by the project include calculation error that occurred while creating a centralized data. There was more reliance on manual methods rather than choosing new technology. One of the major reason for the problem was changing from the U.S. model to U.K. model that first seems to be an easy task but was not. Other reasons that contributes to the problem was tunnel leaks, infrastructure collapse, and high amount of operational cost.

Inappropriate planning of the project was the reason for not being able to manage the problems. It became difficult to manage the issues because there was difficulty in anticipating the risk.

Major issues that resulted in project fauilure include inadequate planning, inappropriate audit checks, no risk assessment done, improper tracking of problems, and improper governance.
