Chapter 22, Problem 3CCS
35-Year-Old Female with Chest Trauma
Barbara Joley was in a major car accident. When EMS managed to free her from the wreckage, she was cyanotic and apneic (not breathing). Her heart was still beating, but her pulse was fast and thready. The emergency medical technician reported that when Barbara was found, her head was cocked at a peculiar angle and it looked like she had a fracture at the level of the C2 vertebra. The following questions refer to these observations.
3. What is pneumothorax and why would it cause the right lung to collapse? Why hasn't the left lung collapsed?

The body of a 19-year-old male is found on the roadway, alongside a vehicle that hit a tree. Three of the four doors of the vehicle have popped open. 2 other bodies are also found at the scene. On examination at the Morgue, the decedent above referenced is found to have minor abrasions of his chin and right cheek, and his jaw appears slightly distorted. However, upon palpation of the mandible, it is found to be grossly unstable, with subsequent X-Ray demonstrating 19 distinct fractures of the jaw between the Right and Left Mandibular Rami. At autopsy, his trachea is found to be filled with vomitus. No other injuries are demonstrable, although there is some fluid residue found in the stomach, smelling of alcohol. a) Given these findings, what are the possible causes of death? Which do you believe to be the most likely cause of death? b) Is it obvious from the injury pattern where the decedent was sitting in the vehicle? If not obvious, where do you believe it likely the decedent was…
After falling off a horse, Mary complained of pain on the right side of her thorax that intensified when she took a deep breath or coughed. The emergency medical technician suspected a broken rib. Using palpation, how could the EMT identify the precise location of the injury?
JL, a 50-year-old woman, fell and broke the left tibia at the ankle. She is in the emergency department, waiting for the fracture to be immobilized. The leg hurts and she notes that the ankle is red and swollen. A diagnosis of a simple fracture and sprain (damage to ligaments) is made.
Describe the pathophysiology of her pain and swelling as related to both the fracture and the strain

He was fully conscious at the scene, complaining of pain in his abdomen and left leg. Examination by paramedics revealed an open fracture of the left tibia with moderate bleeding, left upper quadrant pain with rebound tenderness, and a fast, weak pulse. The leg was immobilized, oxygen was administered, an IV started, and he was transported to the Trauma Center, approximately 10 minutes away. About 3 minutes before arriving at the Trauma Center, he suffered cardiopulmonary arrest from which he could not be resuscitated, despite vigorous advanced life support. At autopsy, his stomach was found to be full of beer, along with 4 whole wieners (without any teeth marks!).
