When discussing, Ms.T noticed that the patient has trouble concentrating on what she was talking. He seemed to be concerned about his incision or wound. Mrs. L tells the nurse that “his office has been laying off workers” that why he is concerned.

A diagnostic statement that contains three parts of the nursing process is known as PES format. The acronym “PES” that stands for “problem, etiology and signs or symptoms”. The clinical manifestations, signs and symptoms of the disease is known as defining characteristics.
