A patient in ICU with an indwelling medical device is presenting with invasive infection
symptoms, specifically fever and chills. They have been on a course of antibiotics that
has not alleviated the condition.
a) What type of organism do you suspect is causing the infection and why?
At autopsy, the most prominent findings were bronchopneumonia with focal organization and hemorrhage in the right lung. Stains of the lung tissue were negative by Gram, methenamine silver, and acid-fast methods, but Dieterle silver stains revealed short bacilli. Lung cultures yielded Gram-negative bacilli, which grew aerobically on buffered charcoal-yeast extract, but not on blood or chocolate agar. The organisms resembled Legionella, but failed to stain with immunofluorescence conjugates for Legionella pneumophila and multiple other species (L minded, L longbeachae, L gormanii, L dumoffi, L bozemanii). The organism was sent to the Centers for Disease Control and Prevention, where it was eventually identified as a new species of Legionella. What is the most probable source of this man's infection?
Family member
Water
Food
Insect
Bioterrorism

Your child has had a moderate fever for 2 days. On the third day, you take her to the pediatrician. After an examination, blood is drawn and a CBC with diff is performed. How does this information aid the pediatrician in determining whether the cause of the infection is viral or bacterial?

- If a patient suspected to have sepsis or meningitis, samples for bacterial testing should be taken before giving antibiotics. Explain why?arrow_forwardTwo patients, with identical medical histories are in adjacent hospital beds. Each is infected with Streptococcus pneumoniae. Patient A's infection progresses to pneumonia while Patient B's infection resolves quickly. The microbial stain of the lung fluid from Patient A indicated the bacterial cells had a capsule, while Patient B's lung fluid stain did not. Why did the capsule affect the course of the infection in Patient A?arrow_forwardChoose the false statement below. O Having Helicobacter pylori as part of the normal body flora decreases the likelihood that a person will develop esophageal cancer. O The normal skin flora of a newborn baby delivered by Caesarian section is the same as that of a baby born via vaginal birth. Babies born via vaginal birth are coated in a protective acidic film created by microbial fermentation in the mother's vaginal canal. Question 19 Choose the false statement below. O A reservoir differs from a fomite as the reservoir is a place of microbial growth. O An asymptomatic host of a microbe may be considered a reservoir if there is no disease state present. O Carriers of a disease show no symptoms nor do they shed microbes passing the disease to someone else.arrow_forward
- A 40-year old man complained to the hospital of fever, chills and abdominal pain. Based on clinical history, the man has elevated ALT and AST enzyme assay with gradual hepatomegaly. The patient submitted stool sample and the following are the results: Many cysts seen with 4 nuclei Eccentric karyosome observed Ingested RBC present Bacteria (Moderate) What possible parasite the patient has been infected with? Explain why. What possible additional lab tests are necessary to prove the patient’s diagnosis? Explain why. What is the possible diagnosis of the patient? Explain why.arrow_forwardA 65-year-old woman has a long-term central venous catheter for intravenous therapy. She develops fever and subsequently has multiple blood cultures positive for Staphylococcus epidermidis. All of the S. epidermidis isolates have the same colony morphology and antimicrobial susceptibility pattern, suggesting that they are the same strain. In what possible way do you think she has contracted the bacteria? Which link in the chain of infection has broken in this particular scenario and what will be your advice to avoid such infections?arrow_forwardPart A) What barrier was breached by the pathogen? Part B) Describe how that barrier works and how it can prevent mom's pathogens from infecting the fetus. A tiny 1-kg (2.2-pound) female neonate (newborn) was born two months premature. The baby had extreme difficulty breathing and had to be intubated (a breathing tube inserted). The mother, at the time of admission, had complained of mild diarrhea and abnormal abdominal pain unrelated to her pregnancy. The infectious disease doctor who was called in to consult on the case immediately recognized the likely problem and ordered blood cultures be performed on the infant. The infant was also started on intravenous antibiotics. Two days later, the lab reported finding a Gram-positive bacillus-Listeria monocytogenes-in the infant's blood. This same organism was the cause of the mother's diarrhea. The mother had unwittingly ingested some unpasteurized cheese contaminated with this pathogen and developed listeriosis. The organism entered the…arrow_forward
- A farmer working on a piece of machinery gets his shirtsleeve caught in a moving piece of the equipment. His shirt is sliced, and a sharp blade covered in mud slices through his upper arm. He attempts to control the bleeding and immediately seeks medical attention. After 3 days, he develops a fever and his arm becomes extremely swollen and painful. Pulling back the bandages, he finds that the wound has become blackened and is leaking bloody fluid. Microscopic analysis of the fluid reveals the presence of gram-positive bacilli. a. Discuss what condition the patient is suffering from and the likely causative agent of this infection. b. Explain how the patient contracted this pathogenic microbe and what virulence factors contributed to the pathogenesis seen at the wound site. c. In addition to antibiotics, the physician prescribes hyperbaric therapy.
