Case History: A patient is admitted to the Emergency
Department with a 3 day history of severe coughing
and fever, then is transferred to the ICU with severe
bacterial pneumonia (lung infection) caused by
Streptococcus pneumoniae. The patient is placed on
the fluoroquinolone antibiotic Levaquin (R) and
initially improves, however the patient relapses and
dies on day 5 of hospitalization. Two cultures were
performed at day 1 and 5. The isolates are identical
with the exception that the S. pneumoniae cultured
from day 1 has a point mutation in the parC gene and
is susceptible to Levaquin while the S. pneumoniae
cultured from day 5 has point mutations in both the
parC and gyrA genes and is resistant to Levaquin. The
patient dies on day 5 from pneumonia and sepsis
