Filip is an 85-year-old gentleman who migrated from Poland with his wife Lena and infant daughter Anna more than 50 years ago. Filip had a successful career as a civil engineer in Australia while Lena worked many years in the State Library. They were both brought up with music around them with Filip becoming a competent violinist and Lena an accomplished singer. They retired about 20 years ago, and became keen gardeners, growing fruit and vegetables in their backyard. They became regular theatre goers with a group of friends, and attend Sunday mass at the local Catholic church. Filip has been struggling with short term memory loss and was diagnosed with dementia 5 years ago. Lena is on a waitlist for right total hip replacement surgery due to osteoarthritis. Lena contacted the Aged Care Assessment Team (ACAT) to discuss transitioning Filip and herself to aged care residential living as a couple. Filip was present for part of the meeting when ACAT visited their family home though he did not speak. Fairview Age Care Home offered respite care for Filip while Lena is undergoing surgery and rehabilitation in hospital, and while waiting for the completion of renovations to the couple’s permanent room in the facility in a month’s time. While Lena was receiving rehabilitation post-right total hip replacement, Anna found her father experiencing increased confusion in the facility. The staff of Fairview Home shared the similar concerns with Anna about Filip’s cognitive status. Filip was admitted to the local hospital for assessment. A Chest X-Ray result confirmed fluid on both lungs with a diagnosis of pneumonia. A urine analysis and other clinical observations showed Filip was mildly dehydrated.
