You have worked at this agency for five years as an entry level case manager and are hoping to be promoted to a senior level position. This non-profit agency assists individuals who have been incarcerated and have recently reentered society. The job description for this position requires the Senior Case Manager to assist clients with finding permanent housing, clothing, counseling services, life skills development, educational opportunities and finding employment. To help you better prepare for the interview, you have been given interview questions a week before your interview by your employer. Your employer wants you to write out your answer to each question and back up your responses with solid evidence. 1. Please describe your educational background and how it will prepare you for the job of Senior Case Manager? I have been a CNA for 15 years. I have worked in many different areas in healthcare and as a mentor. I also just received my associates degree in human and social services. I feel that my educational background has prepared me on different aspects of the human and service world. I have gathered experience in medical administration, and human science. These skills include assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients health and human services needed. I have 15 years of direct care with clients residents so I have an understanding to their needs and where to apply my research for the needs they are requesting.
2 2. What essential skills and workplace capabilities do you possess that would make you a good fit for this position? Please give examples of how you would use these skills as a Senior Case Manager. 3. Summarize why working as a Case Manager is important in service coordination. The ability to assess, plan, implement, monitor and evaluate actions required to meet the clients Health and Human services needed. I possess great communication skills, other workplace capabilities I possess dependability and tolerance. I have showcased the ability to adapt any given situation with communicating efficiently with my colleagues and clients. As you recognize the dignity, worth, and rights that belong to each of your clients, you can instill a sense of self determination in each of your clients as you guide them. This will empower them to reach higher levels of life satisfaction. This will also leave them with the confidence to make healthy choices while you act as the early support system for your client, it is important that you help them to build their own support system and to make their own decisions encouraging the client to reach more effective and sustainable outcomes.
3 The following are situations that you might encounter if you are hired for this position. Please describe how you would handle the following scenarios. Scenario 1: A client has just gotten out of prison and has been in contact with his Parole Officer. This client has been instructed by his Parole Officer to meet with you and get assistance with finding a job. During your first meeting with this client, how would you assess the needs of this client to see if they are ready to obtain employment? What questions would you ask them? What paperwork needs to be filled out during an initial meeting? What skills do they need to be prepared for employment? What other factors could hinder this client in obtaining a job? Please answer this question using research from a scholarly source. During the initial meeting, I will need the client to sign paperwork assuring that they understand what is required of them. For example, remaining crime and drug free. I will explain to them what my job is to them and what my expectations are. I will get to understand them by asking what is their current education level, job experience goals, and skills. I will assure them the required skills needed for this specific employment. For example, basic reading and writing skills. Most importantly I want to establish a understanding that I am here to help them with their transition period I will assess the needs and current conditions my client is in and experiencing, such as financial, physical, and emotionally.

4 Scenario 2: After your first meeting with your client you realize that he is not ready to maintain a full time job. The client informs you that he does not have a stable living environment, is suffering from anxiety and stress, has difficulty reading and writing and has not had a job in 15 years. Describe and demonstrate a plan you would put in place for this client to help them be successful. What support services would you refer your client to? Please answer this question using research from a scholarly source. I will look at areas that may cause stress in my clients daily life and how they can manage them. I will start with emotional and mental well-being, how they deal with stress and illness, their safety and the safety of others, friendship, and relationships, daily living skills, physical health, straight away, i will respect my clients privacy. Some matters may be less important than others and may be discussed at a more appropriate time.
5 Scenario 3: You have set up your plan for your client and they have been following it for the last 3 weeks. How do you determine if your plan is working? What are the best ways to asses if your clients’ needs are being met? Please answer this question using research from a scholarly source. Your client should set both short- and long-term goals. With the short-term goal constituting the steppingstones to achieving the long-term goals. Just as the short-term goals be behavioral, so two should be long term goals you want to ask your client to set. For example, the short-term goal of attending class two times a week and performing a home exercise program at least three times a week my in the long term, increase to attending your balance class two times a week and performing the home exercise program five times a week. Although it is important to set goals with your clients, it is equally important to review their progress towards those goals on a regular basis every week or two during the early stages of a program in monthly or bi monthly as success with achieving goals occurs.) In some cases, you may have to adjust some goals based on your clients progress, health status, and long term goals. Always be prepared to discuss client success as well as struggles towards achieving a certain goal. And that way you can identify what factors help your client meet their goals and then point out these factors during the times your clients are struggling. Finally help your client develop their self monitoring skills by having them keep a journal in which they record their activities they are related to the goals that they set. Making your clients responsible for their own behavior is critical if you want them to continue engaging in behaviors and activities that will lower their risk of falls.(
6 Research three resources in your community that assist individuals reentering society from incarceration. How have these resources/organizations made a positive impact on the lives of formerly incarcerated individuals? Please answer this question using research from a scholarly source. 1. in Ohio the reentry support centers provides a central location where any Ohio resident who has served time for a felony conviction can receive the support services needed to transition back into their communities and the workforce services provided to the participants with training to move forward and succeed in life 2. Saint Vincent de Paul society St. vincents house provides interim housing and support services for formerly incarcerated men returning to the community from Ohio prisons. Residents are both broadly diverse as the neighborhoods from which they come. Program participants come to St. vincents 2 find a safe environment in which they can develop skills to rebuild their lives and reshape their future. Luther social services of Ohio reentry services for returning citizens in a multi faceted program that supports people released from prison returning citizens as they transition back into the community. Reentry services connect returning citizens to needed support and assistance and helps them successfully adjust to living back in their community. All of the core components are designed to provide transitioning individuals with knowledge and experiences a support system and a stable life.
