HSE 310 Family Care Plan Template Client Name: Riverez Family Human Services Practitioner Name: Tiarre Crawford Date Problem Statements  3/31/24 Both parents are high school dropouts who became parents at a young age. Between the chronic conflict with Julio and Quianna’s parents and Julio’s family moving to Florida the couple lack support on both sides of the family. They must rely on each other for support.  3/31/24 Moving away from family to live on their own the couple moves to Englewood, IL. Englewood is known to be a dangerous high crime neighborhood that is plagued with poverty, violence, and unemployment. The family experienced firsthand dangers and violence as Julio was shot in the chest in front of his family amid a gang war that ignited in front of their apartment. The family feels trapped and helpless because they have no means to provide a better living situation. The family is continuing to struggle emotionally as they have not delt with the trauma of the situation.  3/31/24 Although Julio is working two jobs, Quianna is unemployed, and the family faces financial difficulties which have resulted in their inability to provide the essentials for the family. The parents have a hard time keeping food in the house and utilities on as they were shut off in the beginning of April for nonpayment. Also due to the couple’s financial strain Julio must walk to and from work. Julio’s language barriers have inflicted stress within the family causing his connection and communication with his family to become very strained; he has a hard time expressing himself as he is not strong in speaking the English language. 3/31/240 Since the birth of her youngest child Qianna has been displaying signs of depression and she feels helpless as she is left alone to deal with the children on her own. She expresses that she is always tired and not feeling her best self. Women are more at risk than men in developing depression. Depression is a common mental disorder that involves constant feelings of sadness, hopelessness, irritability, feeling tired or low in energy, problems sleeping etc. (World Health Organization, 2023). The effects of the environment, poverty, family dysfunction, and trauma are starting to have a negative impact on their oldest son Rico, for he lacks guidance, discipline, structure, and stability. He feels that he will only end up like his parents
    Proposed Interventions What are the least restrictive and intrusive interventions that you recommend for this family? 1. Individual and family counseling/therapy, support groups 2. Applying for government aid until they are more finically stable. 3. Accessing emergency food banks/ pantries, programs 4. Seeking medical care concerning mental health issues & issues related to Julius injuries. 5. Utilize community resources to find more suitable /affordable housing. 6. Explore community language/ cultural centers where they may offer free English and Spanish classes. 7. Find a mentorship, youth groups, after school programs, etc., for their son Rico. 8. Referral for continuing education (GED classes) for both parents 9. Utilize community job centers. 10. Seek childcare for their non-school aged children. Proposed Strategies Which systemic family strategies best align with the identified needs and strengths of the family and their level of resilience? Getting the family into individual and family counseling could help them to deal with the traumas they have experienced in their current situation. This can help the family process their feelings, develop coping skills, build the family’s connection, and better their understanding and communication skills. Individual counseling can help each member of the family get to the root of their individual issues. Finding support groups within their community can help Quianna deal with the stress, develop new skills and strategies for dealing with her family, feel more confident as a woman and parent, allows her space away from chaotic life, work out her issues with people who can relate to her etc. Having the family visit their local Department of Health and Welfare agency to apply for government assistance/aid in lure of their finical struggles can help relieve some of the stress on the household. The family can apply for the Supplemental Nutrition Assistance Program (SNAP) which would provide the family with monthly food benefits so they can afford food. “SNAP plays a critical role fighting hunger and malnutrition and promoting economic wellbeing” (Center for Study of Social Policy, 2023). Visiting the agency can also help them to enroll in government medical programs designed for low- income families to help them maintain their physical, mental, emotional health at little to no cost to the family. These things will take some of the strain off the family’s already limited finances. Childcare can also be set up for the family. Enrolling in GED classes can help both Julio and Quianna further their education as well find better employment opportunities. While visiting local job resource centers can assist Julio in finding suitable transportation to get to and from work making it easier on his physical issues. Attending English and Spanish classes at local cultural centers or anywhere that may offer free classes can help with the family’s communication with one another.
Finding a mentor and or youth program for Rico can help him to stay in school, better his attitude and stay out of trouble. It will give him an outlet to express himself, work on his delinquent behaviors, learn new skills, give him positive reinforcements, etc. Available Resources What resources are available that you would recommend to ameliorate the identified needs of the family? How could the applied interventions and resources alleviate the family’s needs? These interventions and resources can make all the difference for the Riverez family. Helping the family to strengthen and change their family dynamic in which they can acquire the necessary skills and strategies to provide and maintain better health and well-being, increasing the quality of their lives. Giving the family hope, confidence and promise of a better future for Quianna, Julio, and their children. 1. The family receiving individual and family counseling can bring the family closer together as they learn to deal with personal individual, family, and community issues. Qianna can receive treatment for her mental health issues so that she can feel better within herself which allows her to take better care of her family. Rico can address what causes his delinquent behaviors and learn healthier behaviors. Julio as well as his family can deal with traumas from the shooting

Julio can learn English whereas he will be able to better communicate and connect with his family. 6. Quianna and Julio can get the community support they need. 7. Educate themselves on how to deal with conflict and dysfunction. 8. Better the family’s mood and outlook on life 9. Become better parents. 10. 1 To determine the appropriate resources and interventions for the family I utilized ecomap and culturagram assessment tools to analyze the family needs and strengths. 2 The family is in need of immediate help because they are at great risk of being stuck in current situations and in a dangerous environment. 3 The strengths of the family are that they are all still together, they have love for one another, they want better for themselves, and they haven’t given up on trying to make a better life for them and their children. Their weaknesses are that although they are all together, they lack communication, boundaries, social and family support, resources, and finances. 4 The assessments gave me a blueprint to establish a clear picture of the family’s strengths and needs. Helping me to quickly identify the dynamic of the family’s problems so that I can better understand and identify what interventions and resources that are necessary, relevant, and appropriate in getting the family the help they need.
