PSYC 460:001 Counseling Processes and Techniques ___ Tour 6: Treatment Planning, Progress Monitoring, and Documentation Clinical Treatment Plan 1. Introduction: a brief introduction to the planned modalities, frequency, and expected length of treatment Who referred the client? This will provide insight regarding the client’s motivation. Inform the client of the therapeutic approach generally used by the professional. Explain the general length of treatment. Ethics are discussed: A. Confidentiality and Privileged Communication B. Their participation and agreement to treatment via Informed Consent C. HIPAA 2. Client Goals: these are the key elements of all treatment plans, client goals are unique to each client and describe what behaviors, thoughts, feelings, or interactions will be either increased or decreased as a result of treatment. Should be clear, concise, and measurable 3. Interventions: each goal includes two to three interventions that describe how the client plans to achieve these goals using the counselor’s chosen theory/techniques 4. Diversity and Client Perspective: Progress Notes / Documentation S: Subjective - client’s account of their situation/experience/progress O: Objective - counselor’s observations/description of the season A: Assessment - counselor’s thoughts about progress towards the goals
P: Plan - focus of the next session, homework, follow-up, prepare for next session Document with the following qualities… six “P”s: Ethics In this scenario, we will be discussing a leadership team of directors overseeing a Christian camp and retreat center. All of the directors live on the site that the camp resides in. Therefore, in both summer and retreat seasons, the said directors spend a significant amount of time together. Due to this, they became more susceptible to becoming a family dynamic instead of functioning as a business-focused group. During the most recent summer, one of the directors had been falling short in their responsibilities and had been letting out their frustrations and anger onto the staff of the facility. After a couple of weeks, staff began to report this behavior to the directors and they began to take notice. When the director was approached by the others, they informed the individual of the discrepancies they were worried about and began to make corrective action with this team member. Where issues arose were that the team member began to ignore and almost avoid the advice she was being given to ensure she was okay and being successful, as well as the rest of the staff. She would refuse to actually talk about the issues and would physically walk away any time that they were brought up. When the issues were brought up with the director another time she began to share her side of the story. Through her story, the other director was able to get more information about the rising issue. She stated that she feels quite overwhelmed with her responsibilities but does not want to hand off her tasks because she does not trust anyone else to
be able to get them done the way she wants it. Finally, she finished by adding that she didn’t want to solve the issue because then it would make her feel not as important anymore. This raised concern in the other director as it felt like there would be no way to come to a solution and it may lead to the dismissal of the female director, who the other director still cared for deeply, despite any frustration.
