CAREGIVER RESOURCES & NEEDS DOMAIN Supervision: ( level of monitoring and discipline to promote positive behavior ) Does the caregiver set appropriate limits on the individual? Does the caregiver provide appropriate support to the individual to meet the caregiver’s expectations? Does the caregiver think they need some help with these issues? Rate 0: caregiver has good monitoring and discipline skills Rate 1: generally provides adequate supervision, but is inconsistent; may need occasional help Rate 2: very inconsistent and frequently absent; needs assistance to improve supervision skills Rate 3: unable to monitor or discipline the individual; requires immediate and continuing assistance; individual is at risk of harm Involvement with Care: ( participation level and ability to advocate ) How involved are the caregivers in services for the individual? Financial Resources: Does the family have sufficient funds to raise or care for the individual? Rate 0: sufficient financial resources Rate 1: some financial resources; history of struggles Rate 2: limited financial resources Rate 3: no financial resources Residential Stability: ( current and future housing circumstances ) Is the family’s current housing situation stable? Are there concerns that they might have to move in the near future? Rate 0: stable housing Rate 1: relatively stable; moved in the recent past OR might force housing disruption Rate 2: unstable; moved multiple times in the past year Rate 3: homeless; periods of homelessness in the recent past Medical/Physical: ( prevent or limit ability to parent or care ) How is the caregiver’s health? Do they have any health problems that limit their ability to care for the family? Does the caregiver have developmental challenges that make parenting/caring for the individual difficult? Rate 0: No Rate 1: has developmental challenges but does not currently interfere with capacity Rate 2: interfere Rate 3: Impossible Organization: Do caregivers need or want help with managing their home? Do they have difficulty getting to appointments or managing a schedule? Do they have difficulty getting the individual to appointments or school? Rate 0: well organized and efficient Rate 1: has difficulties (forgetful about appointments OR occasionally fails to retire calls) Rate 2: moderate difficulty organizing and maintaining household to support needed services Rate 3: unable to organize and support Safety: ( within the household ) Is the caregiver able to protect the individual from harm in the home? Are there individuals living in the home or visiting the home or visiting the home that may be abusive to the individual? Rate 0: safe and secure; no risk from others Rate 1: safe; history; others might be abusive Rate 2: in some danger from one or more individuals with access to the home Rate 3: immediate danger from one or more individuals with unsupervised access Family Stress: ( impact of managing the individual’s behavioral and emotional needs ) Do caregivers find it stressful at times to manage the challenges in dealing with the individual’s needs? Does the stress ever interfere with ability to care for the individual? Rate 0: able to manage the stress of individual’s needs Rate 1: history OR suspicion; some problems Rate 2: notable problems; interferes with their capacity to provide care Rate 3: unable to manage the stress Marital/Partner Violence in the Home: ( difficulties or conflict in relationships ) How are power and control handled in the caregivers’ relationship with each other? How frequently does the individual witness caregiver conflict? Does the caregivers’ conflict escalate to verbal aggression, physical attacks or destruction of property? What level of support does the caregiver need to address their problems at work or school? Does the caregiver need support in funding employment or attending school? Rate 0: gainfully employed/ in school Rate 1: mild problems with functioning; needs to be monitored and assessed further Rate 2: moderate problems; difficulties with learning; history of frequent job loss; recently unemployed; need an intervention Rate 3: severe degree of problems; chronically unemployed and not attending any education program; immediate intervention Legal Involvement: ( justice system; legal issues related to immigration ) Has the caregiver been arrested? Is one or more of the caregivers incarcerated or on probation? Is one or more of the caregivers struggling with immigration or legal documentation issues? How does the caregiver’s hesitancy impact their engagement in care for the individual?
