To Whom It may concern, My name is Brianna Harrison, and I am writing to request PLAR for my FWK-3501-01 Field Placement 3 & Seminar. I am using my previous experience as a volunteer Social Services Worker at Chartwells Long Term Care, where I completed a total of 240 hours from January-April 2023. Apply principles of interviewing and counselling to facilitate helping others. As a volunteer Social Service Worker at Chartwells, I have completed self-assessment forms and gathered information about new residents to make an All About Me form for their files by interviewing them or their families. I communicated clearly with residents when they were upset or needed someone to talk to by doing one-to-one visits. During my one-to-one visits with residents, I counselled. I could promptly assess and respond to their needs by observing their body language. I created a safe and non-judgmental environment where residents felt comfortable discussing their troubles with me. I showed them respect by listening and empathizing with their situation. I also provided advice and support, connecting them with relevant resources if necessary. After our conversations, I followed up with the residents to check on their well-being and ensure 1
any issues were addressed. This enabled my residents to trust me and feel comfortable opening up. This was the key to helping them navigate their issues and find the most effective solutions. I also considered their needs and goals and provided personalized advice tailored to their circumstances. I believe I have built good relationships with the residents & staff at Chartwells. I’m very empathetic towards the residents and always make sure that they are okay. Develop strategies and plans that lead to the promotion of personal and professional growth. During my time at Chartwells LTC, developing strategies and plans that lead to the promotion of personal and professional growth. I was able to participate in The Java memory care program. Java memory care is a program dedicated to those living with middle-late dementia. During this meeting, I learnt about how residents become lonely, which increases the chances of getting dementia. I helped residents complete self-care by doing things they like or enjoy doing. For example, one resident finds that reading, colouring, or doing art activities calms them down when they feel upset or overwhelmed. I have also been able to help the residents by building goals for themselves to achieve in their everyday lives to keep them more motivated. While at Chartwells, I was able to participate in self-evaluations that were given to me and accept support and feedback from my supervisor and staff. During my time at Chartwells, I have taken constructive criticism well and accepted all feedback. I have also made sure to always ask questions when I’m unsure of something and arrange times to meet with my supervisor. 2
Communicate and apply agency philosophy, policies, and procedures. Firstly, I ensure all rights are upheld then I ensure that I foster a safe and secure atmosphere each shift by ensuring all resident information in the field is required, for example, appointments, meetings, family visits, outings within the home, and checking the Point Click Care 24 hour summary report to see if any accidents or incident happen during the night I need to know about. I comply with the procedures that are outlined in the handbook when I first started. This relates to COVID, taking your temperature when you come in, putting on a clean mask, swabbing and testing yourself before each shift. When it comes to the ethical standpoint, when working with residents within the home I promote inclusiveness and diversity by making sure everyone feels welcomed and belonging when they’re around me and know/understand that I protect and maintain the confidentiality of every one of my residents. Lastly, professionally, I feel like I create a good rapport with each resident I encounter within our agency. My agency’s philosophy is about making people’s lives better by providing healthier, happier, and better life experiences for seniors that can provide peace of mind for their residents and loved ones. When I first started at Chartwells LTC, I was communicated with the agency’s philosophy, policies, and procedures. To have a clear understanding of what these policies, procedures, and philosophies were, I took my time to carefully read through the agency’s mission statement and asked any necessary questions if I was unsure of the meaning. While communicating the agency’s philosophy, policies, and procedures to others, like residents and staff members, I made sure to be clear and precise by using language that was easy for them to understand and avoided any jargon or technical terms that may confuse people. resident, I recorded what we did/talked about on a database called Point Click Care. This allows the staff to look back and see what types of interactions a resident may need. Maintain relationships that adhere to professional standards that align with social service work. While being a volunteer Social Service Worker at Chartwells LTC, I demonstrated respect, empathy, effective communication, boundaries, professionalism, teamwork, and self-care and I’ve learnt a lot along the way. I have always shown up on time and treated everyone, including staff, with the respect and empathy that they deserve. I was clear and respectful when communicating and listening to others. I worked well with the staff and respect their roles in the home (I didn’t try to overstep). I focused on the needs of the residents and involved them in my decision-making process. I also maintain professional boundaries when it came to ethical standards and completing 1-1 with residents. Lastly, I learnt how to take care of not only the residents but myself by preventing burnout and maintaining my well-being. I always show up and solve problems and identify the causes of what and why the situation at play caused this outcome. For example, a resident had a behavioural issue, so I had to take the proper procedures to first calm her down by assessing what the issue is and then resolving it. In the end, I was able to resolve the issue and calm the resident down by myself. I was very comfortable at placement and was able to enjoy my time here as well as working well with the staff in the home areas. 4
Identify strengths, resources, and challenges of individuals, families, and communities to help assist in achieving their goals. One of my residents has a goal to stay motivated every day to help her stay active and mind-free from any distractions she may have. I have supported my resident by providing her with a hand to help by completing activities with her, painting her nails, talking, and playing cards. I have also made sure to supply any materials that she may need for her room, i.e., colouring books, and arts and crafts. At the end of each visit, I had with the resident. I made sure to report back to my supervisor and record any interactions I had with her. I also let my supervisor know what we did as well as brought up any concerns I had about my resident’s health and work as a team to come up with a solution on how to help the resident. I have spent a lot of 1-1 time with this client and have been able to build her confidence and mood by educating her with proper self-care techniques by teaching her to keep a journal where she can write down her mood, what she did in a day, how she may be feeling, etc. My resident has become more confident in sharing her feelings and speaking about how she feels instead of bottling it up inside. We have also gone through a couple of other self-care techniques that have helped her with these processes. A couple of these techniques include breathing, exercise (walking), relaxing activities, as well as positive gratitude. In my everyday life wherever I go, I can demonstrate this learning outcome as well and I have achieved to excel in this goal at my placement. In a time when needed, I can observe, analyze, interfere if needed, communicate, and solve the problem. 5
Maintain clear and appropriate boundaries between personal and professional relationships, following professional, legal, and ethical standards of practice In my first year of placement, I learned about professional boundaries and the importance of disclosing information. It is my policy not to disclose any personal information about my personal life to my clients or residents. In all my communications with residents and staff members, I use professional language. As a Chartwells employee, you are required to follow the following guidelines, understand your professional responsibilities, respect confidentiality by focusing on work-related topics and avoiding personal discussions at work, avoid conflict of interest, set boundaries, respect your superiors, and finally, seek advice from peers or professionals when you encounter boundary issues. By following these guidelines, you can maintain clear and appropriate boundaries following professional, legal, and ethical standards of practice. At Chartwells, after we complete an activity, entertainment, one-to-one visit, or anything that relates to engagement with the residents, I always report on the iPad with a website called Point Click Care or Activity Pro. This tracks the daily engagement one resident completes in a day. This helps measure the progress and effectiveness of the activities we do with the residents. Additionally, it allows us to reflect on the effectiveness of our work and make any necessary adjustments. It also helps us ensure that all the residents have access to quality activities and services. 6

At Chartwells, they prioritize creating safe and supportive environments. They recognize differences such as physical abilities, sexual orientation, race, ethnicity, religious background, age, gender identity and expression. Employees are respected, accepted, and celebrated for their unique skills and perspectives. This kind of inclusion and acceptance creates trust and a sense of belonging that leads to increased engagement, creativity, and collaboration. It also helps to create a workplace culture that encourages diversity and values everyone for their unique contributions. At Chartwells, and in my daily life in the community. I can and have demonstrated to maintain confidentiality and personal and professional boundaries. I have completed this and keep demonstrating this in placement and throughout my work experience as a Social Service Worker. Communicate and document relevant information according to the agency’s philosophy, policies, and procedure As mentioned previously in the following outcomes, during and after each activity, interaction, entertainment, or outings any resident engages in, we are responsible for documenting the behaviour and engagements that the resident completed that day. For 7
example, when we went to Neb’s Fun World for our bowling outing, I had to make a list of every resident’s name and mark down if they participated and the level of participation. It marked this down in Activity Pro. Now, if there were any behavioural issues, medical concerns, or problems with eating (if they didn’t eat or ate something), we would then be responsible for documenting all the necessary information on Point Click Care. At Chartwells, I participated in outside meetings with other groups. I completed a Zoom meeting with the JAVA program and learnt about new ways to engage the residents. Volunteer experience, I helped set up tons of fundraisers to help raise money for the residents who may not be able to afford to go on outings with the other residents. I have also helped respite a 25-year-old woman who has a disability and within the time I helped with respite I demonstrated confidentiality, giving one-on-ones for self-care giving her goals to complete and helping her with her schoolwork after school. I helped with planning activities and outings as well. While doing respite, I helped her complete a goal based on eating healthier and getting more exercise during the day. She struggled with overeating and didn’t like being all that active, and she wanted to change that. To help her, we sat down and made a goal chart. This goal chart included several activities that could help her stay active during the day that didn’t involve being on her phone or in her room watching tv. Of course, we planned time around the activities so she could have her time to watch tv and be on her phone but, most of the activities involved, outside chores that may be needed to be done around the house. For example, cleaning the garden, racking leaves, taking the dogs for a walk, and meal prepping. 8
After 2 months of setting several goals, she was able to say she felt 100% better about herself and was able to see improvements along the way. Implement the Life Skills methodology across a wide range of environments, supporting individuals’ growth in various contexts. While being at Chartwells, I have been able to complete activities with the resident and complete engaging activities to encourage people at work to reach their daily goals. On my last day of placement, I was able to facilitate my activity. During my group dynamics class, I successfully facilitated a group on the topic of anxiety. Here, I was able to demonstrate a handful of interviewing techniques. Manage compliance with placement documentation in a manner that aligns with timely documentation standards in the field of Social Service Work My professionalism and timely completion of assignments, documentation, and reports have been demonstrated during my four months of placement. All my journals have been submitted on time, and my midterms and finals have been passed. While at Chartwells, I followed the ethical requirements requested by following the documentation standards of social service work and promptly documented information after interactions or interventions. All my work was completed in clear and objective language, I followed guidelines and confidentiality when discussing my day-to-day work in my journals; I kept all documents and reports organized and easily accessible, and finally, when I was completing daily reports using Point Click Care & Activity Pro, I double-checked my work as well as collaborated with my colleagues to ensure 9
