Principles and Power in Leadership
Introduction
Balancing systemic organization objectives in a healthcare setting has become more challenging for leaders in the last decade. Since the beginning of COVID-19, effective leadership
has identified innovative alternatives in providing healthcare services. Reflecting the many demands of health reform, the pace of change is so rapid that many healthcare leaders have left the field, and many health professionals are considering whether to follow (
Porter-O'Grady, 2014). However, some individuals are born to lead, while others learn from mentorship and experience. These are some characteristics of a strong leader apart from a supervisor or a manager. Every successful leader intends to inspire others to reach the organization's goals. A leader takes a more active role versus a passive approach to pursue the organization's success. The purpose of this paper is to focus on the leadership principles and guidelines needed for leaders to thrive. Some concepts in leadership are the fundamentals to inspire and motivate others to achieve the ultimate organization objectives.
Key Details of Chosen Leader
The leader of focus in this project is Jennifer Pacilletti, JFK Medical Center's Advanced Heart Failure Nurse Practitioner (APRN) in Atlantis, Florida (Nurse Practitioner in Atlantis, FL
, n.d.). Jennifer began her journey as a registered nurse with JFK Medical Center cardiovascular intensive care in 2009. In 2013, she advanced as an educator after obtaining her Master of Science in nursing education. In 2016, Jennifer became a ventricular assist device coordinator, where she developed policies and procedures to better help people before, during, and after therapy. In 2017, she became the first APRN in advanced heart failure at JFK Medical Center after completing a Post Master's Certification in Adult-Gerontology Advanced Practice
3
Registered Nurse. Later that year, she became an asset to the left ventricular assist device program (LVAD) by evaluating and advocating for candidates who met the criteria to obtain an advanced mechanical life support device. According to the Centers for Medicaid and Medicare Services (2021), there are only five credentialed and certified LVAD centers in Florida. Leader's Alignment to the Four Universal Principles-Integrity
Integrity is being honest and truthful and is considered one of the fundamentals of higher standards. However, according to Enwereuzor et al. (2020), conducted by professionals in the field of leadership, people who lack integrity also lack the moral conviction to lead effectively. Additionally, despite expertise, intelligence, and appeal without integrity, they're destined to fail as leaders. Integrity and honesty are crucial for an organization to grow. Individuals who hold on
to their principles maintain accuracy based on their beliefs and values, no matter the price. Organizations can rely on leaders with integrity and are admired for their example
. During COVID-19, the ECMO device was the new normal, and ECMO-trained nurses significantly departed. Yet, Jennifer provided ECMO clinical education to oncoming staff, making ECMO a form of ongoing treatment at JFK medical center. Potentially, fewer lives were lost during covid-
19 due to the clinical education and expertise Jennifer provided staff. According to the West Palm Beach local news report by Ryan Hughes (2022), a Tennessee police officer with COVID-
19 was flown to JFK Medical center and received life-saving treatment with an ECMO device in Palm Beach County. Responsibility
Responsible leadership may be described as overseeing an institution's contacts with the public to satisfy the needs of the company's stakeholders and improve the organization's current, social, and environmental standings (Tefera et al., 2019). Moreover, a leader is a mediator for the

patient, the staff, and the organization. Being a resource for quality decision-making is essential in a position of authority, especially in a leadership role where life-changing choices influence human life expectancy, hospital program accreditations, and staffing productivity. Speaking out to facilitate these expectations toward corporate executives is needed to prevent unethical demands. Therefore, a responsible leader can handle all situations and fulfill their commitments. Aside from being truthful, Jennifer displays the fundamentals of a responsible leader. Jennifer provides certification courses for Extracorporeal membrane oxygenation (ECMO), Left advanced ventricular device (LVAD), and other mechanical heart devices to staff and physicians.
ECMO is a temporary automated support system that pumps and oxygenates blood outside the body to allow the heart and lungs to rest due to trauma, injury, and or inability for lungs and heart to function. In 2021, JFK medical center was awarded the
Silver Level ELSO
Award from the Extracorporeal Life support organization. ELSO is an international nonprofit consortium of healthcare centers and individuals dedicated to developing, evaluating, and improving extracorporeal membrane oxygenation (ECMO) and other innovative therapies to support failing organ systems in infants, children, and adults (JFK Hospital and Emergency Rooms n.d.).
Compassion
Compassionate leaders mitigate by helping those who can't help themselves, such as alleviating suffering. One way they show organizational compassion is by protecting their peers from the negative aspects of high-stress demands and caring for others' well-being. Empathy emphasizes sensing the feelings of others. Trust, commitment, active listening, reducing anxiety and fear, and using holistic and creative thinking toward yourself and others are intricate parts of compassion.

Jennifer reveals a geniality to patient care and individualized treatment plans and provides support willingly. When people are diagnosed with end-stage heart failure requiring advanced mechanical heart support, the process is just as complex as the illness. It entails lengthy guidelines and criteria such as insurance approvals, a robust family support system, extensive aftercare follow-up appointments, continuous dietary modifications, lifelong treatment complications, and ongoing education. According to the Centers for Medicare & Medicaid Services (n.d.), chronic end-stage heart failure (New York Heart Association Class IV end-stage left ventricular failure for at least 90 days with a life expectancy of fewer than two years) is not candidate for heart transplantation and must meet specific criteria for an LVAD. Jennifer is involved every step of the way. Individuals and families are vulnerable and afraid since the patients are very sick and can no longer care for themselves. Jennifer provides an atmosphere for
the patient and the families to give feedback and comfort. All patients and family members found
the LVAD information frightening (Schmier et al., 2019). Forgiveness
Forgiveness is a vital principle since human error is inevitable. Forgiveness promotes feedback when an error occurs, which is necessary for identifying a root cause analysis in our healthcare system. Creating an organization of forgiveness allows others and leaders to reveal their imperfections without fear of retaliation. Jennifer displayed forgiveness towards one of her patient's family members. An 18-year-old patient with two small children, newly placed on an LVAD, had extreme family dynamics. Unfortunately, the patient expired, leaving her children with a father who was a known drug addict. However, Jennifer, the day before, spoke with the patient and was able to expedite a living will for her mother to have custody of the children if she
expired. The children's father came to the hospital to pick the children up while he was inebriated

and was very volatile towards Jennifer. Instead of Jennifer retaliating back, she sat with him and showed compassion by comforting him. Thankfully, the children were kept safe with their grandmother due to Jennifer helping get the paperwork completed on time. She oversees the medical and holistic goals of the patient and family.
Bases of Power
Expert power is based on the ability to influence people through your expertise and knowledge. In terms of specifications, control is necessary to persuade others to follow the organization's objectives. One influence of power is the capability to affect others negatively or positively. Jennifer has demonstrated her expertise as an Advanced Heart Failure nurse to her colleagues and the organization. Jennifer participates as a clinical educator for staff and a mentor
for upcoming cardiothoracic nurse practitioners. According to ABIOMED (2020), Jennifer spoke
as a representative for ABIOMED educating medical physicians and registered nurses on an escalation of therapy in cardiogenic shock with the use of advanced mechanical heart support devices. The heart failure program has changed due to Jennifer's approach and expertise toward all parts of the team. We have met the goals and requirements of having an LVAD program because she constantly motivates us and tells us she appreciates our contributions. A leader handles the team successfully for the company (Singh, 2021). Demonstrated Beliefs
Jennifer has demonstrated her dedication and beliefs in the heart failure program, as shown during the infancy stages of her career as a registered nurse in the cardiovascular intensive care unit in 2009, to her role today as an Advanced Heart Failure Nurse Practitioner. Since starting her career, she has focused on improving the heart failure program by dedicating her time, expertise, and academic research. She is beginning with her accolades contributing to

the growth of the heart failure program within the organization to advocate for patients needing an LVAD. She continues to provide clinical education services to JFK employees and specialty certifications. She is a mentor and a resource for new and senior nurses and medical interns by providing continuous clinical feedback. In addition, she offers clinical education services with ABIOMED to the healthcare community to improve patient care and outcomes. Jennifer emulates respect for her professional role, serves her community, and is admired by her peers.
The Leader Effect on Culture
Jennifer's leadership impacts the nursing culture and future nurse practitioners. Through her compassionate care, knowledge, and professional leadership, she has provided an example of
a higher standard of practice. She is respected and highly regarded by her peers and organizational leaders. Jennifer exhibits ethical leadership and is involved in heart failure innovations vital to the advancement of the program. She is a culprit in clinical education as a preceptor for Nurse Practitioner students and cardiology fellowships. JFK Medical hospital is a teaching hospital that offers a medical and nursing residency program. She mentors students during cardiology rotations. JFK Medical Center has received three American Heart Association Achievement Awards for implementing quality improvement measures that ensure cardiovascular patients receive efficient and coordinated care, ultimately leading to more lives saved, shorter recovery times, and fewer returns to the hospital (Cfelixcpa, 2021). Conclusion
In summary, leaders in an organization are motivated and respected for their principles bringing a higher level of work ethic. In addition, Jennifer has aspired to provide excellence recognizing clinical education in advanced heart failure and leaving the ventricular mechanical
8
device to nursing and medical staff. She has proven to be an expert leader by using her knowledge to help educate the healthcare community internally and externally. Leaders who exude assurance are better able to bolster morale, motivate followers, break down communication barriers, and foster collaborative environments (Žydžiūnaitė, 2018).
