What are nursing interventions for a patient with a new onset of acute dyspnea?
If a patient has dyspnea, it is helpful to encourage a breathing technique called pursed-
lip breathing which can result in improved air exchange and decreased dyspnea. It helps the patient control the rate and depth of their respiration, and aids in relaxation to reduce feelings of panic. To instruct the patient- have the patient sit upright, inhale through nose while counting to three, then exhale slowly against pursed lips (Taylor 1503).

If a tracheostomy tube becomes accidentally dislodged, what action(s) should the nurse take initially?
Refer to using the emergency equipment that should always be at bedside, which includes: obturator, suction equipment, oxygen, manual ventilation bag, spare tracheostomy tube of the same size and one smaller size. Then, notify provider (Taylor 1518).

What other actions should the nurse take?
Further asses patient’s respiratory status and keep maintaining patient’s oxygenation throughout the process. Suctioning can be used to maintain airway and provide comfort during respiratory distress (Taylor 1518).

Describe the use of a Dry Powder Inhalers (DPI). a.
Review instructions for the specific DPI to be administered. Remove mouthpiece cover and load dose while activating inhalation. Have patient breathe out slowly without breathing into the DPI. b.
Instruct patient to place their teeth and lips sealed around the mouthpiece.
Patient should breathe in strongly and deeply through the mouth for longer
than 3 seconds. Remove inhaler from mouth, and have patient hold breath
for up to 10 seconds, then exhale slowly. Wait 1 to 5 min for next puff. c.

Describe Metered Dose Inhalers (MDI). a.
Shake inhaler, remove mouthpiece covers from MDI and spacer. Attach MDI to the open end of the spacer. If no spacer is used, hold the inhaler 1 to 2 in away from mouth, inhaling slowly and deeply while depressing the medication canister.
b.
The purpose of a spacer is that it helps deliver the medication to the airways of the lungs instead of the mouth. c.
Have patient hold breath for 5 to 10 seconds, then exhale slowly using pursed-lip method. Wait 1 to 5 min before administering next puff.

Describe the use of Nebulizers. a.
Open device by removing nebulizer cup and place premeasured dose of medication in the bottom of the cup. Use dropper if needed and add prescribed fluid for dilution if necessary. Screw the top of the nebulizer in place and attach cup to the nebulizer. Attach tubing to cuff and to an air compressor or oxygen source. Turn on compressor and notice mist being produced. b.
Instruct patient to inhale slowly through the mouth while holding each breath before exhaling. Nose clip may be used if patient is breathing through the nose. c.
