2 Identifying and addressing quality improvement practice gaps is crucial in any practice or organization to ensure continuous improvement and enhance patient outcomes. One way to uncover these gaps in clinical settings is by systematically reviewing evidence- based procedures and guidelines. A case in point is undertaking a comprehensive peer-review appraising contemporary studies and journal articles in an effort to highlight those aspects where the practice is falling short of the benchmark. My DNP project's quality improvement practice gap would be how reducing hospital readmissions can reduce heart failure. The number of heart failure-related deaths is alarmingly high in studies, suggesting poor treatment (Widegyist et al., 2021). Therefore, focusing on reducing readmissions can go a long way in improving patients’ health and bringing down healthcare costs. Two Types of Tools and/or Methods to Use 1. Implementation of a care transitions program: Kowalkowski's team presented a proposal in 2019. Their strategy is as follows: they use evidence-based protocols; they educate and support patients continuously; and they make sure patients are prepared to take care of their own health requirements after being discharged. With the correct support, a heart failure patient care transition program can reduce the number of recurrent hospital stays. 2. Quality improvement teams: Hedderson and colleagues (2019) proposed interdisciplinary team building to improve heart disease therapy. Nurses, pharmacists, and doctors can all have roles to perform. They would identify weak points, implement focused action programs, and monitor outcomes. It results in ongoing learning inside the company, raising the bar for patient care overall. For example, medication mix-ups when patients transition between care settings are a common problem that these teams may discover. Resolving this requires developing a strict
3 medication reconciliation routine that guarantees that drug lists remain accurate and up to date, which is an essential first step toward closing systemic gaps where errors can happen. To sum up, it's critical—a systematic strategy is required to identify and close gaps in quality improvement initiatives. A thorough review of the literature could highlight these shortcomings, which could then be successfully addressed by quality improvement squads working in tandem with care transition programs. These kinds of programs are essential for improving patient outcomes and establishing a long-lasting culture of progress inside health- focused businesses.
