Dental Hygiene Examination. - 1. Computer simulated clinical exam (CSCE)
2. Patient treatment clinical examination (PTCE)
What is the procedure for contacting CDCA staff for questions with registration? - Contact via
email, questions MUST be submitted in writing
**www.cdcaexams.org/contact
What are the three required qualifications for eligibility for a candidate for the ADEX? - 1. The
candidate has successfully completed the requirement computer simulated exam (CSCE OSCE)
selection with a score of PASS-75% or greater
2. The candidate has successfully completed the patient-based (PTCE) section of the ADEX
Dental Hyigene Examination with a score of PASS-75% or greater.
The candidate has graduated from a dental hygiene program which has been accredited by the
Commission on Dental Accredidation (CODA)
What is the 18-month completion rule for successfully completing both the patient-based and
computer-based exams? - Candidates have 18 months to complete the ADEX Dental Hygiene
exam. The 18 months begins on the date of the first exam taken. If a candidate does not
successfully complete all parts of the ADEX Dental Hygiene Examination in that time, all parts of
the examination including the computer-based portion must be retaken
Explain the three-time failure rule - Candidates failing one or more of the same parts of the
ADEX Dental Hygiene Examination on three (3) successive attempts must re-take ALL parts of
the examination, including any computer-based portions. You must then reapply for the exam.
What qualifications for acceptable photos and the purpose and use of photos during the exam?
- Wearing professional attire or clinically acceptable scrubs. A current passport-quality photo is
required. This photo will be printed on your ID badge for wear at all times during the exam.
What is the timeframe for payment of the initial registration fees? & what is the result of not
paying the fees on time? - Submit payment for examination fees by using VISA or MasterCard
credit card ONLY and paid in full
***Failure to pay the initial registration fees within 72 hours will result in the candidate being
automatically dropped from the exam for which the candidate has begun registration.***
Discuss Application Process Adjustments
1. late applications
2. changes to date or time
3. fee deferrals and refunds/fees applied for a candidate who withdrawals from the
examination both prior to and after the registration deadline. - 1. Late applications may be
accepted depending on available space. Applications received after the published deadline may
be assessed a late application fee which must be paid online within 72 hours.
2. Changing computerized exam date or time must reschedule or cancel their appointment at
least 1 calendar day prior to their scheduled date to avoid being charged an additional fee.
**1-5 days prior to the scheduled test date, a $50 administrative fee will be charged to the
candidate.
***6-29 days prior to scheduling a $25 fee is charged by Prometric
3. request to move examination date to a future date after the registration deadline will be
charged a $100 fee and will have their paid exam fees applied to a future examination.
Candidates must contact the CDCA at least 72 hours prior to the first exam day.
**Refunds for clinical exams who request a refund prior to the registration deadline will be fully
refunded within 10 business days. Candidates requesting to withdraw after the registration
deadline will have a $100 administrative fee deducted from their refund.
What are the 3 requirements for a candidate who experiences a change in his/her name. - 1.
ensure that all of their IDs reflect their new names
2. Once the 2 forms identifications have been changed to reflect the new name, a name change
document must be uploaded in the candidate's online profile.
3. Once the correct documentation is uploaded, you must contact the CDCA to indicate that you
are requesting that your name be changed and that you have uploaded the documentation to
your profile.
What is the purpose and procedure for a Special Accommodations Request? - These are subject
to approval and must be approved CDCA prior to scheduling your exam with Prometric
1. . A doctor's/testing note on his/her official letterhead (or with official stamp) that explains
the candidate's condition and what testing accommodations are requested must be uploaded
to the candidates online profile (without this you will be automatically denied)
2. Once the correct documentation is uploaded, you must contact the CDCA to indicate that you
are requesting special testing accommodations
Explain the procedure by CDCA/ADEX for release of a candidate's score. ex time frame, method
of informing - Scores of all ADEX examinations are released to candidate profiles within 7
business days, but normally released within 24-48 hours following completion of the exam on
your candiates profile
**Results are released to dental hygiene schools and allows for opportunities to improve
curriculum and remediation for the candidate
Explain the remediation requirements of CDCA for re-examination of either the computer or
patient treatment examinations. - CDCA does not require candidates to do remediation, but
candidates need to contact their licensing dental board to determine what type of remediation
requirements they must complete prior to re-challenging any procedure
What is the protocol to follow in the event of an exposure incident? - 1. Immediately clean with
soap and water.
**Mucous membrane exposed to blood or OPIM should be extensively rinsed. All percutaneous
exposures to blood and OPIM should be reported immediately to the chief and the person in
authority at the examination site so that appropriate measures in accordance with school policy
can be initiated and the exposure documented.
What is the protocol that must be followed if two candidates are to share one patient? -
Sharing is not recommended but allowed.
-Each candidate must complete their own med history form and patient consent form for the
patient.
-Must have her own set of qualifying radiographs in order to attempt the exam, copying
radiographs is highly recommended over re-taking radiographs.
**Patients who require antibiotic prophylaxis premedication must be treated on the same day,
not subsequent days.
What is considered an 'eligible' patients - · At least 18 years of age
· Acceptable health history including BP

What are three things the candidate should share with the patient including possible need for
photographs by an examiner and the required time spent during the PTCE. - 1. Patients, their
teeth, and oral cavity may be photographed by designated examiners during the exam. The
CDCA uses such images only for the purposes of candidate and examiner
standardization/training
2. Patients should expect to spend a minimum of 5 hours participating in the examination
Limited treatment is provided under examination conditions and additional treatment may be
required
Explain the documentation required of the patient's blood pressure, medications,
premedication including date(s) recorded, forms required for documentation and student
initials versus signature. - · 159/94 or below is acceptable without a medical clearance
· 160-179/95-109 is acceptable with a medical clearance
-Prior to presenting the patient for case acceptance, the patient must sign and date the medical
history form where indicated on the second page, the candidate must place her initials and the

sign the form until all examination procedures have been completed and evaluated.
What are the Case Selection requirements for the PTCE and what are the teeth selection
requirements? - Full quadrant with at least 6 natural, permanent teeth and two posterior teeth
from a second quadrant. At least two natural, permanent molars; one must be located in the
primary quadrant; one of the teeth in the second quadrant must be a molar. One of the molars
must have both a mesial and distal contact; another molar must have at least one contact
What are the qualifying calculus requirements needed? - 8 surfaces on any premolar/molar
teeth, 5 surfaces located on M or D of molar/premolar teeth, 3 surfaces located on M or D of
molars
the types of teeth that cannot be chosen for treatment during the PTCE. - ·-Implants
-partially erupted 3rd molars

-Teeth with ortho brackets or bonded retainer
Explain the six steps to the online case selection entry process. - 1. Login to your online
candidate profile
2. Yellow box that lets you know when the online teeth selection entry closes, as well as
provides a link for you to "input teeth."
3. Select your primary quadrant and note whether or not you are including 3rd molars
4. Enter you 2 additional teeth, note the qualifications of your additional teeth selection
5. Once additional teeth are entered you may enter your surface selections. Remember to
consult the current ADEX PTCE criteria chart to ensure that you have met all criteria.
6. Summary page will allow you to review all of your entries. If you need to make any
adjustments, just click on any of the tabs to enter the adjustments. The case selection entry
closes 48 hours prior to the start of the first day of the exam.
Define fully erupted - the entire occlusal plane of the 3rd molar is in alignment with the occlusal
plan of the rest of the teeth
Define Un-erupted/partially erupted and three scenerios that would classify as "unerupted" - -
Soft tissue is covering any part of the occlusal surface
-Soft tissue is to the level of the occlusal surface on the distal
-The occlusal plane is not in alignment with the occlusal plane of the rest of the teeth
Identify seven required instruments for the PTCE - 1. Calculus detection- 11/12 explorer
2. UNC probe- probe marked with 1mm increments from 1-10 mm
3. Reflective front surface mirror, which may be one or two sided
4. Blood pressure measuring device
5. Supplies for anesthetic administration
6. Transport container
7. Hand-piece
Identify which 3 instruments the candidate should have a second sterilized available for the
examiners. - 1. Calculus detection- 11/12 explorer
2. UNC probe- probe marked with 1mm increments from 1-10 mm
3. Reflective front surface mirror, which may be one or two sided
Explain the sequence of the exam:
1. cubicle set-up (3 steps) - 1. Candidate cubicle set-up- enter clinic, take patient's blood
pressure; complete appropriate paperwork
2. Request a CFE -CFE conducts the patient/paperwork approval process, the first step in
evaluation
3. Case section entry and verification with the DSM (take your paperwork, patient, and required
instruments to the DSM location to enter, or adjust, and/or validate your Case Selection in the
electronic system)
Explain the sequence of the exam:
2. pre-treatment evaluation - Pre-treatment elevation- a runner will guide your patient to the
evaluation station; all required forms and instruments, including protective eyewear must
accompany your patient
Explain the sequence of the exam:
3. treatment - 1. Patient returned- proceed with treatment, (treatment time is maximum of 120
minutes; preform treatment, calculus detection and perio measurements during your allotted
treatment time)
2. Candidate findings (DSM)- take your paperwork, patient, and required instruments to the
DSSM location to enter and validate your candidate findings in the electronic system
Explain the sequence of the exam:
4. post-treatment evaluation - 1. Post-treatment evaluation- a runner will guide your patient to
the evaluation station; all required forms and instruments, including PPE must accompany your
patient
2. Patient returned- request a CFE when ready to check out
Explain the sequence of the exam:
5. clean-up and check out procedures - Candidate check-out- gather all paperwork, arrange it in
the appropriate order as listed on the candidate check-out form, and then request a CFE to
certify your check-out
Discuss when a second, back-up patient may be presented for treatment. - If before the patient
is presented to the CFE to be sent to the evaluation state, the candidate determines that the
patient is ineligible, a back-up patient may be present. The work-up of back-up patients is at the
expense of the 4 hours and 15 mins of total time allotment. Once a CFE begins the review
process, the exam has begun and a back-up patient may not be presented.
What is the purpose of the Clinic Floor Examiner (CFE) - available on clinic floor to help
candidates navigate through the exam process. CEF is the first point of contact for candidates
when they have questions. once candidate is ready for pre-treatment evaluation, the CFE will
come to candidates operatories in the order in which they have signed up, reviews all proper
completion, the patient blood pressure reading and the medical history for acceptability for
treatment. CFE also reviews progress form to ensure that proper entries for the anesthetic
record have been made, the CFE may also give approval for local anesthesia. The CFE will
review radiographs for compliance with guidelines. Once the CFE begins the patient eligibility
verification process, the exam has begun and a back-up patient may not be presented
Identify the paperwork on tray that must always accompany the patient to the evaluation area -
· Progress form with appropriate sections completed
· Patient medical history form
· Radiographs
· Cubicle card with all sections completed
Identify the instruments (in a closed, sealed, puncture-resistant container) that must always
accompany the patient to the evaluation area - · 11/12 explorer
· Periodontal probe
· Mirror
· Air-water syringe tip (unless provided by the host site)
What what all be present on patient at all times? - · Eyewear
· Clean patient napkin
· Candidate label on the upper right-hand corner of patient napkin or on a plastic badge holder
Procedure for creating an ID card, how and when the card is disseminated to the candidate -
The photo candidate ID badge you receive at the candidate registration session is your
admission badge to the examination day. The candidate ID badge must be worn at all times on
your outmost garment during the course of the examination.
Describe the types of personal candidate identification required for the PTCE - 1. Two forms of
ID, one ID must be a photo ID and both IDs must have the candidate's signature. Acceptable

military ID, or official school ID. A voter registration card (signed) or a credit card (signed) may
be used as a second ID. If your name has recently changed bring a copy of the marriage
certificate or court document to the examination.
What 2 forms of information the candidate must print from his/her online profile for the PTCE?
- 1. 3-digit candidate sequential number which can be found on your registration confirmation
(available in your online candidate profile). **You may either bring a printout of the registration
confirmation or an electric device that can display your identification information in your secure
online profile.
2. Signed certification of review of the online orientation form
Describe Minor tissue trauma - an injury that is inconsistent with the procedure and is expected
to heal without professional treatment by a dentist or physician. Four or more validated areas
of minor tissue trauma constitutes a major tissue trauma, which is a critical error (automatic
failure) (hematoma; laceration/abrasions that is greater than or equal to 3 mm (no need for
suturing, perio packing, or future follow-up treatment); tissue tag; minor ultrasonic burn)
Describe Major tissue trauma - Laceration or injury (requiring sutures, perio packing, or further
follow up treatment); exposure of alveolar bone; moderate to severe ultrasonic burn (needs
follow-up treatment); flap; amputation of papilla)Any validated area of major tissue trauma
constitutes a critical error. (automatic failure)
Identify the five areas evaluated during Post Treatment Evaluation - Five areas evaluated during
post treatment evaluation is calculus detection, probing measurements, calculus removal (sub
and supra), biofilm/extrinsic strain, and tissue management
What is the check-out procedures that must be completed prior to dismissing the patient? -
When patient returns from post-treatment evaluation the following must be done before
dismissing your patient:
1. Complete two copies of the post-operative agreement form (give one completed copy to the
patient and return the second completed copy in the white envelope at the end of the exam)
2. Sign the medical history form
3. Sign the patient consent form
Once you have completed all forms and are signed gather all paperwork and place it in the
provided white envelope and check out with the CFE
What is the order of taking the examinations - PTCE or CSCE OSCE
What is the timeline for completion of online profile - Profiles must be verified in order to apply
for any exam date, and the profile verification process can take 2-3 business days.
*registration must be completed at least 6 weeks prior to the first day of that exam, or they
maybe charged a $250 late application fee.

ADEX Dental Hygiene Exam without graduation - If enrolled in a program where the CDCA
currently administers an exam, your school's coordinator will manage your proof of graduation.
You do not need to upload any documents to this area.
**if not at the school, a letter from the school on official letterhead confirming that you are
authorized, as well as your expected graduation date
Who can Disqualify and Why? - by the dean of his/her dental school at any time after
certification if the candidate ceases to be a senior student of record or the dean (or designated
school official) determines that the candidate is ineligible for any reason.
What purpose of Site Information Sheet? - emailed to the candidates on the final roster once
the exam has been closed (3 weeks prior to the first day of the exam). These Site Information
Sheets can also be found on the dental hygiene exam calendar page of the CDCA website.
Describe the PTCE Scoring: Minimum for each exam - the candidate must score 75 or higher on
each of the two required parts. State statutes have set 75 as the minimum passing score and
the CDCA is not permitted to round up or accept any score less than 75
What score is required for pre-treatment evaluation in order to continue - candidate has not
accrued enough points to possibly pass with a score of 75 points or greater, the examination
will be stopped and the candidate will fail.
