1.Describe/define the aircraft accident that you selected. This Accident occurred on thursday december 8, 2005 The aircraft involved was a Boieng 737-700 that was a year old, in this accident Southwest Airlines flight 1248 was scheduled to have 2 Stopovers which were Chicago Midway International Airport Chicago,Illinois and Salt Lake city International Airport Salt Lake City, Utha The problems started when the pilot attempted a landing with nearly eight inches of snow on the ground in the area. And The 737 skidded during landing And the nosegear collapse, The plane came to rest on Central Avenue just south of the 55th Street intersection at the northwestern corner of the airport. 2.What were the contributing factors to the accident? One of the primary factors which cause the plane to skidded during landing was the presence of 8 inches of snow at the time of landing, but also the calculations of the pilots and their poor choices when deciding what to do, Some human factors that might have had an impact was the maintenance of the runway and cleaning from snow and also the weather was not the ideals to land or pilot could have waited for better conditions. 3.Was there a breakdown in communication? The communication in the cockpit was limited , the pilots did not communicate between each other , the crew was more limited when it came to landing they didn't have any opinion. The communication between the cockpit and air traffic control was poor , pilots did not ask further questions to traffic control and traffic control did not warn the pilots from the previous reports from other aircrafts , the effectiveness of communication controls was not ideal for a flight and the lack of communication was really bad. 4.Were there issues with crew resource management (CRM)? In a part of the landing the cockpit did work together to slow down the aircraft, but their response time was slow not knowing how to react to the situation and their CRM principles were follow at a certain limit, 5.Were there technical malfunctions or failures? The pilots stated that, based on the calculations that the crew had entered into the on-board performance computer (OPC), they believed that they could land and stop safely.However, investigators determined that the OPC based its stopping margins on two assumptions: that the tailwind would be slower than it actually turned out to be and that the thrust reverses would be deployed when landing. The crew fully rely on the computer calculations. 6.What role did human factors play in the accident?
The NTSB identified the psychological pressure to complete their assigned task as one of the factors contributing to the crew's decision to land at Midway despite unfavorable conditions. The crew was pressured to land that plane maybe because of customer satisfaction. 7.How effective were emergency procedures? In general no action was taken by the crew, but the injured passengers were taken to the hospital. We cannot find a lot of information on how the crew reacted to this scenario. 8.What lessons can be learned from the accident? One of the lessons learned is to not fully trust in the system and do your own calculations to be sure,and also more teamwork to have a clear mind when making decisions. One recommendation is more teamwork and critical thinking when having landings in different weather conditions.
