Ruhala Fall 2020 1) Concrete Level – such as a specific physical source is identified as contributing to failure. In the case of Flight 2120, list in chronological order 10 specific failures that occurred during the short flight. Then specifically list 2 major events that lead to the 10 resulting failures. (12 points) Major failures – 1. Axle blew and tires blew out and causes an enormous amount of heat 2. Fire from tires causes major failure of main aircraft components/equipment 1 Low tire pressure 2 Grind rims causing massive heat put into wheel well 3 This burns through aircraft and starts the fire 4 hydraulic acts as fuel for fire 5 equipment failure 6 Air brake stops working 7 Electrical wire ignites hydraulic fluid 8 Fire burns through main fuel tank 9 When landing gear went down it caused airframe to break 10 Aircraft then burns until crashing into the ground 2) Process Level – List the invalid assumptions, faulty reasoning or flawed execution by the flight crew, maintenance persons, scheduling person. (5 points) The mechanics who first spotted the problem should have addressed it. Next, the pilot said they should fill the tires with air before leave but the manager didn’t want any delays.
3) Values/attitudes/perspective level – What was a flawed value system of those responsible for the flight? (5 points) They were trying to rush a pre-flight check as to not delay the flight. This caused them to make fatal mistakes. 4) Engineering Lessons Learned – List 2 ways mentioned in the video that airplanes are now safer due to this tragic crash? (10 points) It is now imperative to check tire pressures before flights and now there is equipment in the airplane that engages a light to know if a fire is occurring.
