Assume that you are the project superintendent in charge of the construction of a
commercial facility. The project involves a large, high cost LEED Platinum facility with very tight
budget and delivery dates. Additionally, the project is already behind schedule and over cost at
the point of speaking. The structural design documents for the foundation design were prepared
by XYZ Structural Design Inc., a consulting company. Structural engineer A of XYX Structural
Design prepared, designed and stamped the design documents for the foundation design.
However, today due a shortage in the inventory of some structural components for the
foundation work you decide something needs to be done in order to keep the project on line
with time. Further, you believe it will be quite some weeks before new supplies are delivered.
The personnel at Structural Design are not available in the near future to recommend design
changes that may help keep the project on time. Since you have many years of construction
experience you feel very competent in making some changes to foundation design.

Assume that you are a Mechanical Engineer working on the design of a boiler pressure
vessel for a nuclear power plant in west Texas. During the process of selection of the design
factor or factor of safety you chose to rely on factor selection guidelines for general design. A
couple of years after the plant became operational, leaks are detected in the pressure vessel.
The power plant becomes involved in a legal dispute with your design firm over the issue of
improper factor of safety selection in the design process. The attorneys for the power plant
maintain that design codes and factors of safety that are based on the American Society of
Mechanical Engineers (ASME) design guidelines for boiler pressure vessel design should have
been used, rather than the general guidelines.

Ajax Welding Inc. repair structural members that have defects. Brown Construction
Company has used Ajax’s services for many years. Recently Brown had to return several
structural steel members that failed during installation. A meeting was held between the two
companies to help resolve the issue. The meeting included Hugo Chavez, a representative from
Brown, Ralph Walter, an Ajax engineer and Chris Smith, Ajax’s returned goods representative.
Chris sticks with the “official position” of Ajax; which is that
the welding design and process were
all right. However, during the course of the meeting it becomes obvious to Ralph that there is a
problem with the design. He suspects that their novice engineer may have used a less than
adequate factor of safety.
