Locate a web source that describes a failure that resulted from corrosion due to improper aircraft cleaning practices and corrosion control practices. In January 2022, an AS565 Panther helicopter operated by the Israeli Air Force crashed into the water resulting in the death of the two pilots. Upon investigation, they found an internal piece of the engine was corroding that isn’t inspected by regular periodic maintenance inspections. The helicopter was supposed to enter another manufacturer inspection in 250 more flight hours. The corroded piece internal to the engine cause a turbine blade to break and resulted in a fire in the left engine which quickly spread to the right engine. Investigators believe that the fire spread so quickly because the piece was made of aluminum and did not withstand the high temperatures of the engine. They also concluded that the salty environment that the helicopter was operating in combined with inefficient engine rinsing post flight contributed to the corrosion of the engine’s internal components. The IAF chief grounded all of the other Panther helicopters while they were inspecting the cause of the incident and they eventually returned to service after each was inspected to ensure they weren’t suffering the same type of corrosion. In this situation I think that it would be incredibly important to make sure that post-flight engine washes/rinses are done thoroughly and 100% of the time and I would also maybe add a borescope inspection of the internal engine components to a more frequent periodic inspection. The manufacturer should also probably try to find a replacement for the component that is less likely to corrode and is rated for higher temperatures. Fabian, E. (2022, July 3).
