Module 03 Lab Activity - CPT Coding Overview: There are 3 parts to this assignment. In parts 1 and 2, you will need to complete the table below. In part 3 you will use 3M encoder software and provide screenshots that show your work. Be sure you code to the highest level of specificity available based on the details provided in the procedural statements. Part 1: Coding using the CPT Index Instructions: In the table below, read each Procedural Statement (in column 1), enter the correct Main Term and any modifying Subterm (in column 2) and CPT Code, Code Range, or Group of Codes from the CPT Index (in column 3). Steps for Identifying the Main Term and the Code(s) using the CPT Index For each procedural statement, 1. Identify the main term (see “Instructions for the Use of the CPT Index” at the beginning of the Index in the CPT code book). 2. Based on the main term you identified, use the CPT Index found in the back of the CPT code book, to find the initial code, code range, or group of codes for the main term and any modifying subterm. Do not reference the main body text of the CPT code book for this part. 3. Compare the main term and any modifying subterm found in the CPT Index for the code(s) to the details in the procedural statement. Ensure that it is the best match. Part 2: Coding Pathways using CPT Main Text Instructions: Use the code(s) you identified in column 3 to find the appropriate CPT Code from the main body text and enter it into column 4. Steps for Identifying the Code using the CPT Main Text For each procedural statement, 1. Look-up the code(s) from column 3 in the CPT Main Text. 2. Read any additional information provided with the code(s) and compare it to the details provided in the procedural statement. 3. Based on that comparison, ensure you have reached the final code that best matches the procedural statement; if you have not, return to step 2. 4. Never code directly from the Index. Page 1 of 6

After each statement, provide a screenshot showing that you were able to follow the encoder process to arrive at the correct code. Make sure that your screenshot shows the final code.
