Lamar University College of Education Teacher Education (For instructing and learning purposes only) Using technology, analyze the 13 disabilities specified in the Individuals with Disabilities Education Improvement Act (IDEIA). Select one or more of the disabilities outlined. Develop a referral profile for an individual based the selected disability, including the reason for referral and the area of suspected disability. Fake Student Information Name: Sandra Bexton School: A.Maceo Reilly Middle School Date of Birth: 02/01/2012 Grade: 6th Age: 11 years, 6 months Gender: Female Date of Report: 09/02/2023 Suspected Disability/Disabilities: SLD Information about the student (age, grade, school, attendance, etc). Sandra is a 6 th grade, 11 year, 6 month old female student at A.Maceo Riley Middle School. She was diagnosed with dyslexia in 2019 and has been receiving dyslexia services. She lives with both of her parents and has a younger brother who is 3 years old. There have not been any reports of any traumatic situations happening in her life, and there are no reports of hospitalizations or surgeries. Sandra is an overall responsible girl; however, she is not able to
perform grade level task. Due to this, sometimes she shuts down when she is not able to fully understand the material being taught. This puts her further behind. Reason for Referral : Sandra has been referred by her parents and teachers for evaluations due to concerns in the areas of basic reading and math. Sandra has been given accommodations and services through the Section 504 committee for dyslexia. Although interventions have been in place since 2022, Sandra is not showing the adequate growth. The evaluation will be used to see if there are any deficits in any areas of her learning that may require her to need special education services. Type of Evaluation: The type of evaluation will be the Woodcock Johnson IV Achievement. We will look in the areas of basic reading including sub tests letter word identification, word attack, passage comprehension, calculation, M.P.S. Applied Problems, Reading Fluency, Oral Reading, Word reading Fluency, sentence reading fluency and written expression. These tests will determine if there is a deficit in any area of reading whether it be in basic reading skills, passage reading fluency, reading comprehension, or writing skill. She will also be checked in the areas of math calculation and applied problems. Interview Information (parent, teacher Interview, and other Information from school) Research-based intervention (included at least one) The mother reports that this has been an ongoing issue and concern for Sandra. Sandra was diagnosed with dyslexia in 2019 and has been receiving accommodations in the classroom setting. Mother also reports that the student receives private tutoring outside of school once a week. Parent indicated that there were no changes in the home, and that Sandra behaves and is a
respectful child. She has friends, and she adjusts to new situations easily. The teacher reports that Sandra is a hard-working student. She gives a good effort but struggles when working independently. She participated in the NWEA MAP diagnostic assessment and scored within the bottom ten percentile. NWEA MAP is the districts diagnostic screener that provides teachers with accurate, actionable evidence to help inform instructional strategies regardless of how far students are above or below grade level” (NWEA, 2019). That is what warranted the referral for additional testing. It has been reported that Sandra is able to perform all activities of daily living by herself, and her hearing and vision were within normal limits. Purpose of Evaluation: The purpose of the evaluation is to see if the student has any deficits in reading or math that would warrant the need for special education services. Assessment Instruments (identify/describe instruments used for the suspected disability and explain why they were selected): The assessment instruments that will be used for this suspected disability is the Woodcock Johnson IV achievement and cognitive assessments. These tests will be able to get a full scope on the students’ current strengths and weaknesses in both reading and math. “The WJ IV provides an array of scores, raw scores, age equivalent, grade equivalent, standard scores, percentile ranks, relative proficiency index (RPI), and instructional zones” (Mather and Wendling, 2015).
