Introduction
Background: The ASIACAN grocery store case presents a critical situation that requires a thorough understanding and application of workplace safety protocols. Accidents and emergencies can occur unexpectedly, posing risks to the well-being of employees and customers. II.A. Accident Investigation Plan
Purpose and Scope: The purpose of this procedure is to deduce the causes that generate accidents through prior knowledge of the events that occurred, in order to be able to design and implement corrective measures aimed at both eliminating the causes to avoid the repetition of the same or similar accidents, How to take advantage of the experience to improve prevention in the company. The registration and control of the accident rate is also the object of this procedure.
The following will be investigated and recorded: ●
All accidents that have caused damage to workers. ●
All accidents with significant material losses or that imply a stoppage of the process. ●
Accidents/incidents that, potentially or by changing some condition, could have had serious consequences, such as outbreaks of fire, free falls of loads, etc. ●
Others that, in the opinion of the direct command, it is convenient to investigate.
Responsibilities:
When an accident occurs in their section or area, direct command is in charge of acting and giving the corresponding instructions to keep the situation under control and prevent further damage. When the accident involves important cures or casualties, you must report what happened, as soon as possible, to the person in charge of your section. The direct command is also responsible for carrying out the investigation of the accidents specified in the scope of this procedure, which occurred in their area or section, and for sending the results of the investigation to management. In the event that the accidents are of a certain importance or that difficulties arise in the investigation of the causes or in the design of the measures to be implemented, you must resort to the advice and cooperation of the person in charge

must control that the preventive measures agreed upon as a result of the investigated accidents are applied within the established period. The prevention coordinator/prevention service is in charge of advising and assisting in investigations whenever, in a line investigation, the investigator so requires. Likewise, it will carry out the investigation of those cases that, due to their complexity, seriousness, etc., require a specialized investigation. You must also collect accident records and compile accident statistics. The management of the company/Administration must notify the accident to the competent authority and ensure that the measures adopted are complied with. All company workers must collaborate and testify in the accident investigation whenever they can provide relevant information about the event.
Investigation Starting: The investigation will be carried out immediately after the accident once the situation has been controlled within a period not exceeding 48 hours.
All the directors of the functional units where the accidents have occurred must be informed about the measures to be adopted as a result of the investigation. The experiences of accidents at work will be used throughout the company. In this sense, the results of the investigations will be disseminated to the managers and personnel affected by the risks in question. The attached accident and incident investigation form (reference code...) must be completed clearly and in detail to avoid further doubts or interpretations. Each of the sections of the form must be completed by the indicated service or person: Human Resources, Direct Command, Prevention Coordinator, etc.

Accidents with injuries that occurred will be recorded annually in the Accident Record Sheet (code ........

Material agent: object, substance or installation that caused the accident ●
Dangerous condition: technical cause of the accident (coded)
Statistical Control:
The evolution of the accident rate will be controlled, detecting whether the changes experienced are due to a random fluctuation or a new factor that has modified the safety conditions. For this purpose, the monthly frequency and incidence indices for accidents with sick leave and for total accidents (with and without sick leave) will be calculated. They will be represented according to each month of the year.
Official Notification
Accidents with medical leave The official notification of work accidents (reference code ..........
) will be completed, delivering a copy to the administration department, another to the injured party and the rest to the managing entity ( mutual) within a maximum period of 5 business days from the day of the event. In the case of serious, very serious, fatal accidents or incidents that affect four or more workers, in addition to the previous procedure, a communication will be made (via telegram) to the Provincial Directorate of Labor and Social Security, within a maximum period of 24 hours.
Systematic Cause Analysis

It is simply using the basic headings, for example “immediate causes, unsafe acts”. The incident would then be considered against “immediate causes, unsafe conditions”. Unsafe Act: Work on running equipment, Work / task carried out wrongly. Unsafe Condition:
Defective equipment/systems/materials, Fire or explosion hazards, Slippery or rough surfaces, Tight/narrow workspace (possibly).
The next step would be to consider each of the “immediate causes” identified in relation to the list of basic factors, for example: Unsafe Act: >Work on running equipment > Personal Factors - inadequate work standards - insufficient training > Job factors - experience (possibly) - lack of awareness - lack of knowledge This process is then repeated for other unsafe acts and conditions. Once all potential basic cause factors have been identified (often items tend to repeat themselves) then each basic cause is mapped against a control element, sometimes called “area for corrective action”, e.g. employee training, evaluation of loss prevention work (risk assessment) or engineering controls. However in addition it also provides a more in depth analysis of the incident and very much brings the focus of corrective and preventive action back to assessing the fundamental management processes that define the equipment used or the level of training. Developing Corrective Actions Adequate corrective actions to minimise or eliminate a problem can only come from a sound investigation, which has truly solved the problem. Otherwise, the problem will occur again and again but with different symptoms. An action plan must be prepared with timing objectives and names of people (or department) in charge of the actions. Following up the action plan up to its completion is essential.

Report An investigation:
The report should be written in a timely manner. It communicates facts about the incident/accident. It should identify corrective actions. Most organisations have a standard investigation report form. Using a standard form has several benefits. ●
It raises all the questions that should be answered; i.e. what was the event? What happened? What were the causes? etc. ●
It provides consistency in data reported. It prompts sharing of information with others. It allows analysis for trends and that helps safety management.

It provides follow-up corrective actions.
Communication Results:
One of the most valuable actions is to circulate information within and outside organisations in an efficient way. The use of photographs is advised since it draws the attention and sensitivity of people and helps them to remember. The communication actions might be: ●
Immediate:
Safety alerts, training, etc.

Short term: Newsletters, Group meetings, etc. ●
Long term: Changes in Company Standard or Procedures and Retraining programme.
Accident Investigation and Reporting Steps Summary:

Identify what happened and how the incident/ accident could occur. ●
Investigate, analyse, report and record accidents and/or near misses utilising the root cause analysis method and other techniques such as brainstorming etc.

Determine potential areas where current management practices may need further evaluation or

Capture and track all actions; ensure they are clearly identified and implemented. ●
Manage and control, by means of a system providing for a definite assignment of accountabilities (and completion target dates) that each preventive/corrective action step is implemented. Communicate information within and outside the organisation. An internal system should exist and establish that all deficiencies are promptly detected in any other location, plant and activities and that, as mentioned above, suitable preventive / corrective action plans with accountabilities and target dates are enforced to eliminate the identified deficiencies.
II.B. Emergency Response Plan for ASIACAN Grocery Stores
Purpose and Scope
: The purpose of this emergency response plan is to ensure the safety and well-being of customers, employees, and the store premises during emergency situations. This plan includes potential emergencies, such as natural disasters, fires, medical emergencies, and security threats. It outlines the responsibilities, procedures, and resources necessary to effectively respond to and mitigate these emergencies.
Emergency Response Team: The Emergency Response Team (ERT) is composed by individuals responsible for implementing the emergency response plan, including key personnel from different departments, such as store management, security, human resources, and department supervisors.
Emergency Communication: Communication is one of the keys of success. The communication protocol must be clear and ensure effective and timely communication during emergencies, including the chain

of command and alternative communication methods in case of communication system failure. It also includes the installation of emergency communication systems, such as alarm systems, public address systems, and emergency notification systems, to alert customers and employees in the event of an emergency.

Investigation Process: This will explain the steps taken to investigate the incident. This includes collection of evidences, interviewing witness or employees, reviewing security camera footage and consulting any pertinent documentation

describe the sequence of events or conditions that led to the incident using a systematic
approach to guide the analysis.

Emergency response strategies must be tailored to each location and developed with local authorities. These plans should incorporate scenarios and follow local regulations and resources. To test the effectiveness of emergency response strategies, employees should also participate in regular drills to learn the procedures and identify areas for improvement, and they must know their duties and responsibilities to ensure smooth emergency evacuation. Accident Investigation and Emergency response plan reviews and revisions should be performed regularly for continuous improvement and for creating a safety culture both for employees and the customers.
