According to HIPAA, an information release form must include several important elements to protect patient privacy and ensure the proper handling of their medical information. Below are some of the things that I noticed about the Ballad patient release of information form.
1.
HIPAA requires the patient’s social security number to uniquely identify them and prevent any confusion or mix-up with other patients. The Ballad form requires only the last four digits of the patient’s social security number.

HIPAA requires the information release form to include an expiration date to specify the duration for which the patient’s consent is given to release their medical information. I could not find an expiration date on the Ballad release form.
3.
HIPAA requires the doctor’s signature on the form signifying their approval and acknowledgement that they have reviewed the request and are authorizing the release of the patient’s medical information. With the Ballad form I did not find a place for the doctor’s signature; however, they have a place for the team member that is processing the request.

HIPAA also requires that a phone number be included on the information release form allowing the recipient of the form to contact the patient or the patient’s representative for any clarifications or follow-up questions. The Ballad form requires the patient’s phone number only and not the representative.

HIPAA requires that the type of PHI that will be shared or disclosed be described.
With the Ballad form they have a list of requested information to be checked off.

The purpose of the disclosure should be explained before completing the process. In the Ballad form I did not find the place where you define the purpose of the disclosure.

The disclosure should have the patient’s dated signature accompanied by all of the
above requirements. If a patient is having a representative sign on their behalf, you must also obtain a description of their relationship to the patient and documentation of their authority to act on behalf of the patient. The Ballad form has a place for the patient/representative to sign and date the form, and it also requires that the relationship to the patient, if the representative is signing on behalf of the patient, be present.
Overall, the Ballad release of information form seems to comply with the HIPAA requirements. I think that they should add a few more security features to their form but they may possibly get that information verbally.
