The topic of scissor lifts is referenced in this incident. While there are not specific standards for scissor lifts, and an argument can be made for the application of training and the limiting of capacity under the General Duty Clause. Training on the operation of a scissor lift should be offered and manufacturer limits should be followed at all times. 2.
List three questions that would be answered in helping to determine what personal factors were influential to the injured and involved persons. (10 pts)
Why was work continued with and not stopped?
Why was the removed dirt not dropped off before attempting to remove the planter?
What precautions were taken before attempting to remove the planter to keep it stable and free from hitting the scissor lift. 3.
In producing a countermeasure plan for the incident, what efforts are based upon the root causes of this incident? (10 pts)
Make sure lifts in use are capable of handling the load and stress that is going to be put on them. All excess weight that is not needed to complete the task at hand should be removed before completion of work. Proper training, both classroom and hands on, should be completed and documented before beginning to use lifts.
