I found the documents online as well but I must admit I was being picky and combed
through at least 10 websites to find the format I wanted. I think an older adult may
need assistance to get directed to the document that best suits their wants and
needs. Then there was the issue with downloading as a word document. Everything
would want to access Adobe Acrobat in order to get signatures, which I didn't want to
use. I agree having advanced directives in an Emergancy makes all the difference to
make wishes known. reading your post left me wondering if my hospital provides a
way of getting advanced directives started. I found that although we don't have the
paperwork on the unit there is a webpage which allows for patients to get education
and forms provided on advanced directives. I also found it interesting that the
webpage states " Federal law requires hospitals to give adult patients information on
advance directives" (C-HCA,2023). I have yet to be asked by a patient if we can
provide advanced directive paperwork. Thankfully now I know the answer to that
future question.
