A1. Selected Healthcare Organization The healthcare organization I selected is Advent Health Zephyrhills, located in Zephyrhills FL. A2. Significance of Healthcare Organization I chose this facility because I was a nurse tech and became an RN while working for this organization. They are a faith-based organization whose mission is to improve standards of care while promoting faith-based values to those in the community. While employed at this hospital, my brother was in a major car accident. Unfortunately, he did not survive his injuries. I cannot begin to tell you the support I received from the hospital during my time of grief. I was shown love, comfort, and was even blessed by the hospital financially. So I know first hand the values they have and how much community means to them as an organization. A3. Type of Facility Advent Health Zephyrhills is a short-term acute care facility. This hospital offers a wide range of services to the community. Some of the clinical services provided include Emergency care, Cardiovascular/Cardiovascular Surgery, Intensive Care for adults, Comprehensive Stroke Center, Radiology Services, Orthopedic Services, Labor, and Delivery, Wound Care and Hyperbaric Oxygen. Just to name a few. This hospital is fully accredited by The Joint Commission. a. Type of Control Advent Health Zephyrhills is a church-based voluntary, non-profit healthcare organization. This means they receive favorable tax exemptions, lessening the tax burden for the facility, allowing them to use more of the funds towards the overall mission. Those missions are individually designed to match and support the needs of the community; have many stakeholders involved in the process. The IRS is the governing body that determines which organizations receive tax exemptions.
A4. Discussion of the Healthcare Organizations Overall Star Rating Advent Health Zephyrhills has a star rating of 4 out of 5 stars on the Compare Care website. The overall star rating is calculated based on how well a healthcare facility performs across five (5) areas of quality; summarizes quality information on key topics, such as readmissions, infections associated with central lines/foley catheters, and deaths after heart attacks/strokes. The overall rating, between 1 and 5 stars, summarizes a number of measures across 5 areas of quality into a single star rating for each hospital (Medicare.gov, 2023). The five areas of quality currently include: Mortality, Safety of Care, Readmission, Patient Experience, and Timely and Effective care. A4. Additional Source of Quality Data on Healthcare Organizations Quality hospital data can be found online at CMS.gov. Additionally, Det Norske Veritas, also known as DNV Healthcare, Inc, provides quality data on healthcare organizations. Accreditation by the DNV indicates the facility meets or exceeds standards developed to enhance the quality and safety of healthcare. A5. Organizational Ethics Reflected in Facility and Control Type Advent Health Zephyrhills is owned by The Adventist Health System, a voluntary Non-Profit Healthcare Ministry, whose values, and organizational ethics are applied and reflected to promote common good in the communities it serves. Two organizational ethics reflected by the facility include Equity and Social Justice. The organization is committed to improving the health, prosperity, and well- being of the communities we serve. They celebrate the diverse backgrounds, cultures of our patients, visitors, and colleagues, and embrace opportunities to learn and grow from new perspectives. Two upcoming events include an outreach to the community to offer psychosocial support through prayer and encouragement. Another aspect of the event focused on offering blood pressure screenings including education and addressing food insecurities by supplying canned food to those in need.

A6. The Nurse’s Role in Value Based Care Value-based care was designed in response to the dramatic increases in healthcare costs and poor- quality outcomes. Nurses play a key role by identifying high-risk patients and directing process changes utilizing evidence-based interventions. This coordination of care with the health care team, leads to improved outcomes with far fewer complications, leading to a reduction in healthcare costs. Nurse can support value based care by clarifying the need of possibly unnecessary procedures or tests. As nurses we are qualified to coordinate the care for our patients and to advocate accordingly. It’s also important to provide our patients with quality care. An example of this includes turning our immobline and bed-bound patients every 2 hours to allow for adequate pressure relief to prevent the development of pressure injuries. Hospital aquired pressure injuries can affect the organization negatively with reimbursement rates. A7. Organizational Change to Improve Delivery of Value-Based Healthcare An organizational change that could improve the delivery of value-based healthcare is to ensure patient-centered care is being delivered. Identify the patients with the highest risk of readmission and ensure that all resources are offered and involved in a safe and successful discharge with the likelihood of positive outcomes at home. B. Financial Comparison of Selected Healthcare Organization to Nearby Healthcare Organization I chose to compare Advent Health Zephyrhills, a non-profit short term acute care hospital, located in Zephyrhills, FL vs. HCA Florida Citrus Hospital, a proprietary corporate short term acute care hospital, located in Inverness, FL. (American Hospital Directory, 2023). B1. Alignment with Value-Based Healthcare Advent Health Zephyrhills appears to be more aligned with value-based healthcare. When comparing the two facilities on Medicare.gov Care Compare website, AHZ has an overall star rating of 4 out of 5 stars and HFCH has an overall star rating of 2 out of 5 stars, showing a significant difference in
ratings. An overall star rating is based on how well a hospital performs within different areas of quality. Such as treating sespis, heart attacks, pneumonia, the rates from readmission, and lastly safety. AHZ will receive more reimbursement for services. “Patient centered care requires that health care organizations and health care professionals actively understand what patients value. Fortunately, there are methods for gaining that understanding. But, they need to be adopted much more widely, and patients need to be treated as full partners in their care.” Edgman-Levitan, S., & Schoenbaum, S. C. (2021). B2. Alignment with Equitable Patient-Centered Care AHZ is more aligned with equitable patient-centered care by the ratings provided. The patient-care model emphasizes the value of patient views, not only complimenting health-care provider prospectives, but also to provide the nurses insight into healthcare effectiveness. This also includes improvement of patient experiences and outcomes while decreasing healthcare services utilization and costs (Santana et al.,2018). B3. Sources to Inform Organizational Transformation CMS Hospital Compare website is one source of financial data that informs organizational transformation. The website provides information on the quality and performance of hospitals, including patient satisfaction in all different areas of care and the rates of readmissions. HCAHPS survey data shows the quality of healthcare received at the facility from the patient's survey of the care they received. Medicare's Care Compare website in the form of an Overall Star rating for each facility. This star rating system is based on how well a hospital performed across different areas of quality and safety.
