HSA 6108 Community Health Assessment MEMORANDUM To: Sally Riley, MHA, FACHE From: Date: 11/12/2023 Subject: Community Health Needs Assessment Project Dear Ms. Riley. As a follow up to your recent request, please find contained herein a plan for the development of a Community Health Needs Assessment (CHNA). I look forward to your review of my suggested approach and answering any questions you may have. I will wait to hear from you and look forward to our discussion. Broward County Great Hopital will need to develop a Community Health Needs Assessment, implementation strategy according to the CHNA results and make them public to improve the health of our local community and most importantly serve our stakeholders. We are not only required to develop a CHNA every 3 years but to maintain accreditation for our tax-exempt status. This is a general requirement by the Internal Revenue Service under Section 501(c)(3). (IRS 2023) These rules are also mandated by the Patient Protection and Affordable Care Act (ACA). (Lopez et al 2021) A CHNA will identify, prioritize and give what our community needs. It will also be a catalyst to assist the unmet health needs focused on our general public. The key elements that are needed for the development of this project will provide a deeper understanding of our current social status and how poverty can affect health in individuals and communities. (Ndomoto et al 2018) Focusing on the current needs of our local community is
HSA 6108 Community Health Assessment vital because if too much general data is gathered, we can suffer the ill-effects of analysis paralysis. (Walston 2023) Data gathered will be current and hyper-focused on our local status to align with our mission statement and goals as a health organization. To develop our Community Health Needs Assessment, we will need to host stakeholder meetings, create community focus groups, conduct surveys and interview our community leaders and analyze our current health data. These steps will give significant and essential health data to serve our community health needs. (CRH 2023) Holding stakeholder meetings will welcome the inclusive process and engagement. It will define their roles and responsibilities, resulting in their expected involvement to facilitate collaboration. (ACHI 2023) Creating community focus groups will help us learn more about our community’s opinions and interests. Any and every type of relationship needs to have comprehension and understanding of each parties’ needs. There are many benefits to conducting community surveys. The purpose of surveys are to discover the opinions and preferences of a specific population or demographic to increase sales or specific data. This feedback will improve patient experiences and can reinforce the reputation of the healthcare organization which can translate into increased patient volumes and market share, ultimately leading to higher revenue (Marsilio, 2022). The results of increased revenue will benefit stakeholders and the overall outcome of our organization as a business. Lastly, meetings or interviews with our community leaders will facilitate collaboration even further. This will build trusting relationships with individuals and organizations within our community and begin as an inclusive environment that promotes a sense of joint ownership of the CHNA process. (ACHI 2023) The trust this creates between our community leaders and organization will
HSA 6108 Community Health Assessment contribute to better health outcomes of our community, strategically align our shared goals and sustain these relationships over time. To execute our CHNA project, I recommend building and assembling a strong Community Health Needs Assessment Team. This CHNA Team will organize each stakeholder and conduct community leader interviews. We will hire a reputable company to create surveys with our chosen demographic to give live feedback and status of our community. To analyze the data collected, our Six Sigma Team will provide essential data and charts to implement our Community Health Needs Assessment and serve our community. The final results will give Broward County Great Hospital profound knowledge of our community, community demographics, our social determinants, current health needs of local uninsured citizens and prioritize those in need and underserved.
