Milestone One 1 Milestone one Specific gap in the delivery of healthcare The selected gap is based on a gap in quality related to care coordination for individuals with chronic illnesses. United States has the highest number of people suffering from chronic illnesses such as diabetes and hypertension. The specific gap is lack of information management system for managing chronic illnesses among individuals and families. Most chronic illnesses are genetic and there is need to manage even those family members affected by the sick individual. This is an issues that affects all the members of the population. There is no specific population which is mostly affected by this problem but everyone is affected in the country. History of this gap in access to healthcare The United States health system faces significant challenges that clearly show that there is urgent need for reform. There is evidence that Americans often do not get the care they need even though the United States spends more money person on healthcare than any other country. One of the major issues is lack of preventive care which is underutilized. In the past, the number of Americans who did not have health insurance was high but now it has reduced. However, the need for an information management system is high to manage chronic illnesses. This has never been implemented before and people with chronic illnesses have been treated with no good coordination. An information management system could have been developed before to try and manage such cases including the family members of those individuals affected. Milestone One 2 Chronic diseases in USA is already highly prevalent and it is expected to continue being high over the next decades if no action is taken. The burden of the associated physical and economic costs will worsen, as well. But the burdens of chronic disease do not just loom in the future. Evidence suggests that the high prevalence of chronic disease may be partially responsible for the devastating effects of the current coronavirus pandemic. Having a system where all the family members that are affected by a certain chronic disease, it would be easier to manage them since most of them are manageable when they are realized early. How the healthcare delivered to the population is affected by the gap in access. The healthcare delivered is not efficient for the chronic illnesses. In most cases, chronic illnesses such as diabetes and hypertension are recurrent within a family. It becomes very difficult to manage these illnesses. An information system that keeps records of family members affected by those diseases can help in tracking the diseases and treating them early hence reducing adverse impacts of those diseases. Prediction of any potential implications if this gap in access is not addressed If this gap is not addressed, there is a very high possibility that the healthcare systems in the United States will have a hard time addressing chronic illnesses. These illnesses are not easily manageable and when many people are suffering from such illnesses, it becomes difficult to address it. It is therefore necessary that an information management system is implemented to monitor such cases..
