OHS8035/6 Hazard & Risk Evaluation A hazard is a situation, condition or thing that may be dangerous to the safety or health of workers. Risk is the combination of the likelihood of a harm actually happening, and the severity of that harm. To identify and control hazards in a workplace effectively, it is important to evaluate the risk of each hazard. A risk matrix is often used during a risk assessment to measure the level of risk by considering the consequence/ severity and likelihood of injury to a worker after being exposed to a hazard. The two measures can then help determine the overall risk rating of the hazard.

Identify five (5) of the hazards from the photograph of a roof replacement worksite (included on the last page). 2. For each hazard calculate a risk rating using the Risk Evaluation table provided. 3. Enter all of your answers in the Table in the spaces provided.
OHS8035/6 Hazard Risk Rating (LxC) Falls: Lack of Fall Protection from the roof can be the leading cause of injury and death in the roofing industry. 5x5 = 25 Length and Angle of Ladder: A long, heavy ladder can be cumbersome and awkward to handle, increasing the chances of tipping or slipping. It might also extend beyond a stable support point, compromising its safety. Even Angle of ladder is also not proper. The recommended angle is 4:1 (for every 4 feet of height reached, the base of the ladder should be 1 foot away from the wall). 5x4 = 20 Heat stress: Working on a hot roof can lead to heat stress and heat stroke. It is important to stay hydrated, take breaks in the shade, and wear loose-fitting clothing. 4x2 = 8 No Caution Tap Near Work Area 2x2 = 4 Open Windows: Open windows may create unnecessary noise and even may increase the chance of falling objects and more injury to people below. 2x3 =6 Part 2 Based on the risk rating, select the highest risk hazard (only 1), using the risk rating descriptions below, and recommend three (3) controls to reduce the risk and protect the work from the hazard you identified.

OHS8035/6 Risk Rating (LxC) Descriptions 15-25 High Constitutes a Significant Risk. Managing this risk is a priority and additional risk control measures are needed. Interim steps may be needed prior to implementing permanent solutions. 5-14 Medium Constitutes a Moderate Risk with Caution. Investigate if additional measures can reduce risk even further. 1-4 Low Constitutes a Tolerable Risk. Monitoring is required to ensure controls are maintained and effective. Highest Risk Hazard Recommended Controls (List 3) Lack of Fall Protection Always use personal fall arrest systems (PFAS) when working on a roof. This includes a full-body harness, lifeline, and anchor point that meets current OSHA standards. Complete comprehensive fall protection training, including proper use of PFAS, fall rescue techniques, and roof safety protocols, before performing any work on a roof. Install guardrails with toeboards around all open edges of the roof where work is happening.
