CS4005 Social Change Initiative Infographic Outline Use this document to create an outline that addresses important elements of your infographic. [What is the title of infographic?] Using Community Intervention to reduce Cardiovascular Disease Mortality and Morbidity in the United States [What evidence (e.g., prevalence and incidence rates) do you have that the health issue needs to be addressed?] Incidence rates: i.e., one-third of all deaths globally are contributed to CVD and top five leading causes of death in the US [Who is the target population that will benefit from the social change initiative, and how will they benefit?] Those who are from low socioeconomic status and racialized groups; Benefits by directly increasing health literacy [Who are the key partners and stakeholders who will be involved in executing the initiative?] Healthcare providers: for providing Comprehensive Cardiovascular Risk-Management plan and other information related to CVD Community-based organizations/other social institutions such as church: implement workshops to raise awareness on CVD Advocacy groups: collects fundings required for increasing access to healthcare services Pharmaceutical companies: for self-measured blood pressure mentoring loaner program [Briefly describe your social change initiative.] The social change initiative will involve community-based intervention. The community-based intervention involves inducing changes in the lifestyles of the peoples at risk of developing complications from CVD through different workshop and programs. This will involve loaning self-measured blood pressure mentoring system. The communities will work directly with the local clinic, who would need to implement Comprehensive Cardiovascular Risk-Management in order to ensure personalized health care services can be provided to those suffering from CVD.
