I am a registered nurse in the HVI unit. I am calling about a patient name Gracelyn Rodriguez who got admitted with a heart attack. S (Situation) Why did you call or why are you having this conversation? The patint is experiencing shortness of breath and started experiencing chest pain with an 8.5/10 on the pain scale. B (Background) What is the important background information that the person you are communicating with needs to know. The patient got admitted about 12 hours ago. She started of with dizzeness and a light chest pain. She has been diagnosed with heart disease for 3 years. A (Assessment) What are important assessment findings that relate to the reason you are communicating. What is currently going on? Based on my assessment, her symptoms may be associated with a potential heart attack again. R (Recommendation) What do you want? Would you like a EKG done ? and if so, when can you check up on the patient ? 1. How did using the ISBAR communication pattern help you in preparing to communicate the issue with the person you selected to communicate with? It helped me explain the situation better without leaving any details. It helped me by making sure that I had the right recommdation to help the patient. The great way to also use this kind of communication is to help us document it as well.
2. Ask the person you communicated with for feedback on any communication that was unclear, confusing or potentially left out. Share the feedback from the other person here: The only thing that the person I shared it with recommended was to maybe also include vital signs and what kind of heart disease my patient has. Another suggestion was to add more symptoms to be a little bit more specific that applies to the diagnosis.
