He received a smaller dose of pain relief medication that seems to take no action, while patient X still screams and lies in a fetal position while holding his stomach and says his pain seems never to go away.

One of the nurses who are witnessing these situations is just a trainee nurse and feels bad for the child patient, who is screaming in pain. One of the best actions of the trainee nurse is to contact the care nurse who is caring for the patient and can advocate on behalf of him. The nurse can act an advocate for the patient and can help the patient to be relieved with pain. The nurse needs to get attention of the physician to enquire of this situation.
