The nursing extern meets Ms. J a mother of one-year-old girl child who is diagnosed with failure to thrive and admitted in the well-child clinic. The girl child is being on the rehabilitation program and prescribed with a feeding program, since the child do have normal growth and seems to very small in size. When the student nurse visited the patient to take vital, the nurse extern could observe that the patient’s foot is hanging on the edge of the cot.
