A request for a patient’s medical record is sent by fax to your office. The fax cover sheet contains the letterhead of a nearby medical facility. You do not recognize the name of the physician, fax number, or telephone number stated for the physician’s office. How do you respond to the request?

The daughter can be considered an expert witness in a scenario like this because she has knowledge, education, and experience. That being said, her testimony can confirm that there was negligence. Not only that, but if a patient has documentation on a visit they had with that location but they however don't that obviously makes it suspicious which leads to what the daughter stated.
