Summary Introduction

The evaluation is the final phase of the nursing care plan in which the nurse and the patient together measure the achieved goal for a specified nurse care plan. The clinical reasoning and evaluating process consists of five classic elements such as identifying the criteria and standards for evaluation, determine the collected data in which they met with the criteria and standards are not, interpreting and summarizing the findings, documentation, and termination or modification of the plan.
