as chronic distress. Specifically, Mr. Cannoli reported organizing his daily activities around
avoiding situations and environments in which he has felt lightheaded in the past, and feels very
distressed when he can't avoid those situations and begins to feel lightheaded.
BACKGROUND INFORMATION
Mr. Cannoli reported that his fear of situations in which he might faint first began approximately
18 months ago. He reported that he was in a movie theater and began to feel lightheaded, and had
the thought, "What if I faint here? What if no one helps me? I may never be able to get out." He
reported that his fears of fainting and situations in which he feels lightheaded have been
persistent since that time.
PAST AND CURRENT SYMPTOMS
Mr. Cannoli reported fears of both crowds and enclosed places, such as department stores, movie
theaters, grocery stores, driving in a car, and while he is teaching in the front of his class. He
reported that he fears that he will faint in these situations, that he has felt faint, lightheaded, and
dizzy in those situations before, and that he fears fainting approximately 80% to 90% of the time
he is in these feared situations. He reported specifically fearing that if he begins to feel
lightheaded in any of these situations, he might faint before he is able to escape. He avoids
situations such as grocery shopping, movie theaters, and department stores whenever possible,
and finds these situations distressing when he is unable to avoid them. Specifically, Mr. Cannoli
reported that he completely avoids movie theaters and department stores, even when friends
invite him out to these situations, which he feels has negatively impacted his social relationships.
He also reported delaying and avoiding grocery shopping as long as possible, sometimes for
