A 28/ female sought consult in a primary care
clinic because of a 3 day history of fever (max
temperature 38.2 degrees Celsius), sweating and
productive, yellowish sputum.
(+) pleuritic chest pain in the left axilla
(-) episodes of breathlessness
· Employee at the mall, regularly commutes,
exposed to second hand smoke
Fond of eating food at fastfood, does not take
any vitamins/supplements
Lack sleep most of the time
Occasional alcoholic beverage drinker (2-3
bottles/beer during weekends), occasional
smoker (2-3 sticks/day since she was 18 y/o)

presented with one month history of epigastric
pain, described as burning, association with dry
cough, and occasional belching, and sensation
of regurgitation perceived as 'bitter after-taste'
The pain worsens after eating and when she is
lying flat.

Patient also presented with postprandial
fullness. She also wakes up with abdominal pain
and would have hoarse voice the following day.
The patient noted that symptoms become
partially relived by adjusting from a lying to a
sitting position and by using KremilS.
Patient is an employee at the mall and does not
eat on time and would often eat at the nearby
canteen or to the vendors outside her work to
save money.

Fond of drinking soft drinks 2 sakto bottles/day
and 3-in-1 coffee (3 or more sachets/day)
Patient was given Omeprazole 40 mg/capsule

30 minutes per breakfast which afforded relief of
symptoms.
