There are two types of drugs in blood: bound and unbound. Depending on the affinity of a specific drug for plasma proteins, a portion of the drug may become bound to the proteins while the remainder remains unbound. If protein binding is reversible, then there will be a chemical equilibrium between the bound and unbound states, such that:

Notably, it is the unbound fraction that has pharmacologic effects. It is also the fraction that can be metabolized and/or excreted.
