elevated temperature of 39.1°C (102.4°F), chest pain, and
breathing difficulty. Physical examination reveals crackles and
diminished lung sounds over the left lung field. His history
indicates that he has not received routine childhood vacci-
nations. Initial laboratory testing shows an elevated white
blood cell count with 89% neutrophils and elevated C-reactive
protein. Staining of sputum shows both gram-positive and gram-
negative bacteria, indicative of a mixed bacterial infection.
Additional testing is undertaken to identify each bacterium.
