After Creating the DB and the Table described
above, Fill the table with dummy data, then
create a PHP that contains two things:
1. A dropdown list with all doctor IDs from the
DB
2. A table listing all history contents

table dynamically changing with the change of
the doctorID dropdown list. You may use AJAX if
you want.
