Surgical Case # Diagnosis: Clotted AV Graft Procedure: Left forearm dialysis graft thrombectomy and angioplasty Using sterile technique, ultrasound guidance, and a micropuncture set, access was obtained to the arterial side of the graft and a 7-French sheath was placed with the tip directed toward the venous anastomosis. A 7-mm balloon was then advanced across the venous anastomosis, and multiple dilations were performed. The thrombus was removed from the graft using the clot-buster device. Heparin was then injected into the graft. An additional 5-French sheath was then placed in the venous side of the graft with the tip directed toward the arterial anastomosis. A 5-French Fogarty catheter was advanced across the arterial anastomosis and pulled back into the graft. The pulled thrombus was then removed.
