ASSESSMENT PROCEDURES
PREPARATION
1. Introduce yourself and verify the client's identity. Explain to
the client what you are going to do, why it is necessary, and
how the client can cooperate.
2. Perform hand hygiene and observe other appropriate
infection control procedures.
3. Assemble equipment
4. Provide for client privacy.
GENERAL APPEARANCE & MENTAL STATUS
5. Observe for signs of distress in posture or facial expression.

6. Observe body build, height, and weight in relation to the
client's age, lifestyle, and health.
7. Observe client's posture and gait, standing, sitting, and
walking.
8. Observe client's overall hygiene and grooming.
9. Note body and breath odor.
10. Note obvious signs of health or illness.
11. Assess the client's attitude.
12. Note the client's affect/mood; assess the appropriateness of
the client's responses.
13. Listen for quantity, quality, and organization of speech
