![Control Engineer was under great pressure
from top management to "get plant running at
all cost". There was a delay in starting the
plant as scheduled following a turnaround,
because of other software problems. To
isolate the source of the problems, Samuel
made experimental changes that disabled
some of the software interlocks.
When the TO tripped, the production process
continued. There was some confusion in the
control room and the operators could not
decide on the required response. The Plant
Superintendent was contacted and he
immediately ordered the shutdown of the
plant. The plant was finally shutdown 22
minutes after the TO stopped.
A gas cloud containing highly toxic and
carcinogenic gases was blown to the
neighboring school. The school called
wanting to complain about the smell but the
receptionist did not know who to direct the
call to as the managers were busy running
