- History of the problem
The client has been suffering from the feeling of hopelessness, helplessness, and worthlessness for 1 year which is precipitated by failure in marriage. The client thinks that he is responsible for his unsuccessful married life as he was not competent enough to run his family.
2-Prior medication history- The client has been taking the following anti-depressants for the last year.

Group therapy
The client is asked to sit in a group of people where he will feel welcomed and will develop positive changes in himself.The client shows a positive response to treatment.
3-Medical history
The client has no evident medical history. Got serious shock during father’s death since last year due to coronavirus and he is too up set
5-School history
The client did not hold a very good relationship with his peers. He was a remain isolated type of student. He was neither active in academics nor sports.
6-History of friendship
The client did not have good relationships with his peers. He also has marital disputes but an extramarital affair is not there.
The client is currently employed working as a clerk and is not satisfied with his job because of an excessive workload.
The client is oriented to place, person and time.
Sleep
The client has been suffering from insomnia for the last 7 months.
Attention and concentration Attention and concentration are impaired as the client is not able to tell numbers from 100 to 0[backward count].
Perception
The client has been suffering from hallucination as he perceived rope as a snake.
delusion - The client has persecutory delusion as he thinks his wife wants to kill him because of failed married life.
Affect
The client has feelings of hopelessness and worthlessness as he thinks that he is an unwanted material in this world and nobody loves him.
Behaviour-
The client most of the time remains mute and does not want to involve with others.
Mannerism and posture-
The client has a waxy posture and remain seated in one place for hours,
Anxiety
The client feels anxious about his helpless condition and has a fear that he might not does well in his future.
Psychosexual problem- The client has not a psychosexual problem.
Psychosomatic disorders- These are the disease that does not hold any pathological background.
Addiction- The client is not addicted to any substance.
Family psychopathology- In the three-generation, there is no evidence of psychiatric illness in the family.
Personality traits- The client has a passive personality i.e. he feels negative towards everything.
Interview behavior- The client has closed, anxious behavior as he was not responding to questions well.
Insight- The client has 2 degrees of insight which means he is not at all hopeful about him.
Tentative diagnosis- Insomnia
The client likes his reserve type of nature.
Effective coping strategies that client has learned -
Relaxation and meditation will help to gain concentration and reduce stress.
The goal for therapy is time boundation and the goal should be attainable.
